Which medical specialty has the greatest gender imbalance?
In South Korea, the field of urology stands out in this regard. According to studies, over 90% of board-certified urologists in South Korea are male. However, this imbalance is not as pronounced in other countries such as the United States. For instance, the gender ratio among urology residents in the U.S. is approximately 3:1, male to female.
This difference stems from how urology is perceived in each country. In the U.S., subspecialties in urology that focus on female urological conditions, such as cystitis and urinary incontinence, are well established. However, in clinical practice, gynecologists in South Korea often manage not only reproductive tract conditions such as vaginal bleeding but also female urinary complaints. This is largely due to the structure of the healthcare system, where patients have direct and rapid access to specialists. As a result, conditions like dysuria or urinary frequency in women are frequently addressed in gynecological settings, rather than being referred to urologists. As a result, urology in South Korea has become centered around male health issues, and the proportion of male specialists has become overwhelmingly dominant.
However, urology is not a field exclusively for men. Women who experience discomfort during urination or have issues related to the urinary system should consider seeing a urologist, as this is often the most efficient path to diagnosis and treatment. In the past, the prevailing belief was that “men see urologists, women see gynecologists,” but this distinction is gradually disappearing. Consequently, more women are now visiting urology clinics, and many report high satisfaction after treatment—often becoming repeat patients or recommending urology services to their peers.
Urologists are increasingly mindful of creating a comfortable environment for female patients. Since urological conditions significantly impact the quality of life for both men and women, those experiencing symptoms are encouraged to consult a urologist without hesitation.

While this field is sometimes trivialized due to its association with sexual pleasure.
