횡적부조화를 동반한 골격성 III급 개방교합 환자에서 불리한 치주상태를 고려한 절충치료 치험례

국문 초록

목적: 본 증례는 횡적부조화를 동반한 골격성 III급 개방교합 환자에서 불리한 치주상태를 고려한 개별적인 치료계획 수립과 면밀한 안모 평가를 통해 심미적이고 성공적인 치료결과를 보여주었다. 증례: 26세 여성 환자가 전치부 개방교합과 상악 중절치간 정중 이개를 주소로 내원하였다. 임상 및 방사선적 평가를 기반으로 골격적 횡적부조화와 개방교합을 동반한 골격성 III급 부정교합으로 진단하였으며, 상하악 절치의 중등도 치근흡수와 상하악 전치부 및 상악 소구치 치근첨 부위의 얇은 협측 피질골 두께를 확인하였다. 상악에 MARPE를 적용하여 횡적 부조화를 해결하고, 개방교합의 해결에 있어서는 골격성 3급 안모와 심미적인 미소선 형성을 고려할 시 상악 전치 정출이 우선 고려되었으나 본 환자의 전치부 치주적 예후를 고려하여 상악 구치부 함입을 일부 허용하였으며, 하악에서는 전 치열의 후방이동을 계획하였다. 구치부 함입 중 턱끝 돌출과 같은 안모의 변화 및 악궁 확장과 전치부 정출에 따른 치주 상태를 면밀히 관찰함으로써 적절한 전치부 피개와 교합관계, 그리고 심미적인 미소선을 달성하였다. 환자는 치료 결과에 만족하였으며 치료 기간은 총 25개월이 소요되었다. 결론: 골격성 III급 부정교합 환자에서의 턱끝이 돌출된 안모 양상과 취약한 치주환경은 개방교합 치료에 불리하게 작용할 수 있다. 이러한 경우 미소심미에 대한 세심한 분석과 그에 따른 전략적인 치료계획 수립, 치료 중 안모와 치주 환경 변화에 대한 면밀한 경과 관찰이 동반되어야 안정적이고 심미적인 치료결과를 달성할 수 있다.

영문 초록

Purpose: This case report demonstrates an aesthetic and successful treatment outcome through careful monitoring and establishment of an individualized treatment plan for a patient with skeletal class III open-bite with a transverse discrepancy and unfavorable periodontal condition. Case Report: A 26-year-old female patient presented with an anterior open bite and diastema between the maxillary central incisors. Based on the clinical and radiographic evaluations, the patient was diagnosed with skeletal Class III malocclusion with transverse discrepancy and an anterior open bite. We observed moderate root resorption of the maxillary and mandibular incisors, and thin buccal cortical bone surrounding the upper anterior teeth and premolar root apex regions. The maxillary transverse discrepancy was addressed using a mini-screw-assisted rapid palatal expansion (MARPE) device. Maxillary incisor extrusion was preferred for correction of the anterior open bite due to the patient’s reverse smile arc and skeletal Class III profile. However, considering the patient’s poor periodontal prognosis, intrusion of the maxillary molars and subsequent mandibular autorotation were planned to reduce the amount of incisor extrusion. In addition, total arch distalization of the mandibular dentition was planned. Throughout treatment, profile changes such as chin protrusion and periodontal alterations associated with arch expansion and incisor extrusion were closely monitored. As a result, favorable incisor overlap, occlusal relationships, and aesthetic smile lines were achieved. The patient was satisfied with the outcomes of the treatment, which took 25 months to complete. Conclusion: Chin prominence and compromised periodontal conditions can negatively influence the management of open bite in skeletal Class III malocclusion cases. Therefore, detailed analysis of smile aesthetics, strategic treatment planning, and close monitoring of facial and periodontal changes during treatment are essential for achieving a stable and aesthetic outcome.
