국문 초록

This case report represents the successful treatment of a median cleft lip using the modified DeHaan’s technique, which has been shown to provide effective outcomes in achieving optimal aesthetic and functional outcome. A 3-month-old male infant of Vietnamese nationality presented with a midline upper lip cleft, bifid upper labial frenum, widened alar base, an ill-defined philtrum, and a subnasal pit. A modified DeHaan’s method was performed, including a V-shaped philtral flap for symmetry, realignment of orbicularis oris muscle, nostril reconstruction, and subnasal pit excision. At one-week follow-up, a successful outcome was observed with minimal scarring. At four months postoperatively, the patient exhibited stable wound healing, natural philtral symmetry, and improved nasal contour. The modified DeHaan’s method effectively restored philtrum symmetry, nostril sill contour, and columellar height, improving both function and aesthetics. Long-term followup is essential to assess nasal growth and scar maturation. This case report suggests that DeHaan's technique could be a promising surgical option for the treatment of median cleft lip.
