국문 초록

In cases of bilateral complete cleft lip and palate with severe premaxillary deformity, primary vomerine ostectomy is often necessary to achieve proper alignment of the premaxilla for successful cheiloplasty. However, due to the procedure being performed on inherently unstable and malformed structures, meticulous surgical technique is essential to minimize complications. This report presents a case of premaxillary impediment following primary vomerine ostectomy and nasolabial repair in a 10-month-old patient. It is recommended that vomerine ostectomy be performed posterior to the vomeropremaxillary suture through a minimal incision on the vomerine crest, while minimizing manipulation of the surrounding mucoperiosteal tissues. By doing so, stable surgical outcomes can be achieved despite the structural challenges.
