횡적 부조화와 전치부 개방교합 및 안면 비대칭을 동반한 골격성 II급 부정교합 환자의 절충치료 치험례

국문 초록

목적: 개방교합과 횡적 부조화 및 안면 비대칭을 동반한 골격성 II급 환자에서 상악 좌측 제2소구치 발치 및 하악 좌측 제1대구치 발치, 상악 구치부 압하와 하악 전치열의 후방이동을 진행한 증례를 보고하고자 한다. 증례: 본 증례는 39세 여환으로 하악 전치부의 총생과 전치부 개방교합을 주소로 내원하였다. 환자의 주소 외에 상악 우측 제2소구치의 결손과 정중선의 편위가 관찰되었으며 CBCT 검사 결과 상, 하악골의 횡적 부조화도 관찰되었다. 총생과 정중선 개선을 위해 예후가 불량한 하악 좌측 제1대구치와 상악 좌측 제2소구치를 발치하였고, 결손된 상악 우측 제2소구치 공간은 폐쇄하기로 하였다. 환자의 나이를 고려하여 횡적 부조화는 상악골의 골격적 확장 없이 치성적으로 보상하기로 하였다. 골격성 고정원을 이용한 상악 구치부의 압하와 하악 전치열의 후방이동을 통해 개방교합의 폐쇄 및 안모의 개선과 바람직한 수평피개 및 수직피개를 얻었다. 결론: 장안모, 골격성 II급, 전치부 개방교합을 동반한 환자에서 골격성 고정원을 이용하여 상악 구치부의 압하과 함께 하악의 저항 중심을 고려한 전치열 후방이동을 통해 교합평면의 회전 및 구치부 의 정출을 방지하면 성공적으로 하악골의 전상방 회전을 유도할 수 있다.

영문 초록

Purpose: We report a case of a patient with an anterior open bite, facial asymmetry, maxillomandibular transverse discrep-ancy, and skeletal Class II malocclusion. The patient underwent orthodontic treatment, including the extraction of the maxillary left second premolar and mandibular left first molar, which had poor prognosis. Additionally, the treatment involved maxillary molar intrusion and the total distalization of the mandibular teeth. Case Report: A 39-year-old patient visited our clinic for the treatment of anterior open bite and mandibular crowding. She also had a congenitally missing maxillary right second premo-lar, midline deviation, and facial asymmetry with a transverse discrepancy. For crowding and midline correction, the man-dibular left first molar and maxillary left second premolar were extracted, and the space left by the missing maxillary second premolar was planned for closure. Considering the patient's age, we decided to compensate for the transverse discrepancy through tooth movement. Maxillary molar intrusion and total distalization of the mandibular dentition, supported by ortho-dontic miniscrews, led to the closure of an anterior open bite, improvement of the facial profile, and establishment of proper overjet and overbite. Conclusion: In patients with a long face, Class II malocclusion, and anterior open bite, a successful upward and forward rota-tion of the mandible can be achieved by intruding the maxillary molars and distalizing the mandibular dentition with the aid of orthodontic miniscrews. Distalization of the mandibular denti-tion, considering its center of resistance, can rotate the occlusal plane and prevent extrusion of the molars.
