안면 비대칭이 있는 성장기 환자에서 최소 수술을 고려한 악정형장치 및 교정적절충치료 치험례

국문 초록

목적: 중등도의 안면 비대칭을 동반한 성장기 환자에 대해 최소한의 이부 수술을 고려하여 성장조절 치료와 가상 치아 배열을 기반으로 한 교정적 절충치료를 시행한 증례를 보고하고자 한다. 증례: 본 7세 여환은 치아가 고르게 되고 비대칭이 바로 잡혔으면 한다는 주소로 내원하였으며, 과발산형의 골격성 I급, 상·하악 전치부 총생, 그리고 이부의 좌측 편위에 의한 골격성 비대칭 양상을 보였다. 상·하악 총생을 해소하기 위한 가철성 확장 장치와 sectional expansion screw를 첨가한 monobloc 장치를 이용하여 상악 우측 구치부의 탈보상과 비대칭적인 하악골 성장을 조절하였다. 성장 완료 후 고정성 장치를 통한 교정적 절충치료를 시행하였으며, 디지털 가상 치아배열을 통해 필요한 치아 이동 양상을 파악하였다. 하악 전치의 치조골 열개와 round tripping을 방지하고자 협측 미니스크류를 이용하여 분절호선 상에서 하악 구치부의 후방이동을 선행하였다. 적절한 I급 구치관계를 달성하였고, 하악 전치부의 총생이 해소되었으며 적절한 전치의 치축이 형성되었다. 결론: 총생 및 안면 비대칭을 보이는 성장기 환자에서 악정형 치료를 통해 비대칭적인 악골 성장을 조절하고 사전에 시행한 가상 치아 배열을 통해 안전하고 효율적인 교정적 절충치료를 시행할 수 있다. 또한 성장 완료 후 환자의 선택에 따라 비대칭적인 이부 개선을 위한 최소한의 수술을 고려할 수 있다.

영문 초록

Purpose: We report the case of a growing patient with moder-ate facial asymmetry who underwent orthodontic camouflage treatment using a digital virtual setup following orthopedic growth modification with minimally invasive surgery. Case Report: A 7-year-old girl visited our clinic for the treat-ment of anterior crowding and facial asymmetry. She had skeletal Class I malocclusion with anterior crowding, facial asymmetry, and a hyperdivergent profile. To relieve the crowd-ing, a split removable plate appliance with a median screw was planned, and a monobloc appliance with a sectional expansion screw was employed to correct the asymmetric mandibular growth and for decompensation of the upper right molars. At the time of growth completion, orthodontic camouflage treat-ment was initiated using a fixed appliance. The expected tooth movement was confirmed using a digital virtual setup, and distalization of the mandibular posterior teeth was performed on a segmental archwire with mini-screws on the buccal alveo-lar bone. A Class I molar key to occlusion was achieved, and the axis of the lingually inclined incisors improved following ante-rior crowding relief. Conclusion: In a growing patient with anterior crowding and facial asymmetry, asymmetric mandibular growth can be con-trolled through orthopedic growth modification. Orthodontic camouflage treatment can be performed using an efficient treatment strategy based on a virtual tooth setup. Minimally invasive surgery can be considered after growth completion for improvement of chin asymmetry.
