상악 좌측 견치의 구개측 매복과 과개교합을 동반한 성장기 골격성 II급 부정교합의 치험례

국문 초록

성공적인 성장 조절을 위해 치료의 시기는 매우 중요하며, 성장 단계의 정확한 판단과 적절한 치료 방법, 환자의 협조도가 치료의 성패를 좌우하게 된다. 성장기 골격성 II급 부정교합 환자의 경우 상악 성장을 억제하는 구외력과 하악 성장을 증대시키는 악기능 장치를 사용할 수 있으며 2가지 방법을 같이 사용 시 효율적인 치료 효과를 나타낼 수 있는 것으로 알려져 있다.1 또한 상악 견치는 전치부 중 가장 긴 치근을 가지고 있으며, mouth corner에 위치하고 있어 심미적으로 중요하며, 교합 시 canine guidance를 담당하는 등 기능적으로도 큰 역할을 하는 매우 중요한 치아이다. 견치는 발육 기간이 길며, 긴 맹출 경로, 인접 치아 중 가장 늦은 맹출 시기 등으로 인해 이소 맹출과 매복이 쉽게 일어난다. 견치 매복의 2/3은 구개측에 위치하고 있으며, 이러한 매복 견치의 맹출을 유도하여 주는 것은 구강 기능의 회복에 있어 매우 중요하다. 본 증례에서는 상악 견치의 구개측 매복을 동반한 성장기 골격성 II급 부정교합 환자에서 악정형 치료와 매복 견치의 견인을 통해 양호한 치료 결과를 얻었기에 이를 소개하고자 한다.

영문 초록

Treatment timing is essential for the success of orthopedic growth modification, along with the accurate diagnosis, appropriate treatment method and the compliance of patients which determines the success of the treatment. In the case of skeletal Class II malocculusion, extraoral force that restricts maxillary growth and functional appliance to increase mandibular growth can be applied, and it is well known that if two treatments are performed in combination, the results are better and more efficient. Moreover, maxillary canine has the longest root among anterior teeth, is located at the mouth corner, and manages canine guidance which plays an essential role in function as well. The canine has a long developmental period, eruption path and also has the latest eruption time compared to other teeth thus ectopic eruption and impaction could easily occur. Around 2/3 of impacted canines are located at palatal area, thus it is essential to perform guided eruption of the impacted canine to recover oral health. The following case shows a skeletal Class II malocclusion with a palatally impacted maxillary canine treated with orthopedic appliances and traction of the impacted canine.
