국문 초록

뇌졸중 후 인지 손상은 빈번하게 발생하며 이는 재활치료, 현업 복귀를 어렵게 하고 궁극적으로 삶의 질에 부정적인 영향을 미친다. 신경심리평가는 뇌졸중 후 인지 손상의 정도를 파악하여 진단 및 적절한 치료적 개입으로 이어지는데 유용한 정보를 제공할 수 있다. 임상현장에서 사용되는 대부분의 신경심리평가도구는 알츠하이머형 치매를 평가하기 위해 개발된 것이며 뇌졸중 후 보이는 인지 손상의 임상적 특성을 고려한 검사 도구가 부재한 사유로 평가와 이를 활용한 치료적 개입에 어려움이 많은 실정이다. 따라서 본 종설에서는 인지 손상 측정을 위한 신경심리평가 도구에 대한 전반적인 개관과 함께 평가 도구의 특성을 비교하였다. 또한 현재 사용되는 신경심리평가 도구의 제한점과 더불어 뇌졸중 후 인지 손상 측정을 위한 평가도구가 갖추어야할 요건에 대하여 고찰하였다. 마지막으로 해외에서 최근 개발된 뇌졸중 후 인지기능 선별 평가도구를 소개하는 것으로 향후 도구 개발에 대한 통찰을 제공하고자 하였다.

영문 초록

Cognitive impairment occurs frequently after suffering from stroke and it makes both rehabilitation treatment and returning to work very difficult. and has negative effects on quality of life. Neuropsychological assessment could provide useful information that can be used to assess the extent of PSCI in order to lead to the accurate diagnosis and proper therapeutic intervention for the patients. Since most existing neuropsychological assessment tools were specifically designed to assess Alzheimer’s disease. the tool considering clinical characteristics of PSCI does not exist yet; thus, there are many difficulties in conducting accurate assessment and intervention to treat cognitive impairment.. Therefore, in this review, the characteristics of the evaluation tools were compared with an overall overview of the neuropsychological evaluation tools for measuring cognitive impairment. Also, the limitations of those existing neuropsychological assessment tools and requirements of assessment tools in order to assess PSC correctly were reviewed. Finally, it was intended to provide insight into development of relevant future tools by introducing recently developed overseas screening test for PSCI in this review.
