보그트-고야나기-하라다병과 연관된 홍채-수정체 가로막의 전방 편위 1예

국문 초록

목적: 홍채-수정체 가로막의 전방 편위가 선행한 비전형적인 보그트-고야나기-하라다(Vogt-Koyanagi-Harada, VKH)병 1예를 보고하고자 한다. 증례요약: 35세 여자가 우안 시력저하를 주소로 내원하였다. 교정시력은 우안 0.6, 좌안 1.0이었으며, 안압은 우안 14 mmHg, 좌안16 mmHg였다. 구면렌즈대응치는 우안 -17.25 D, 좌안 -9.75 D였다. 세극등 현미경 검사와 3차원 전안부 촬영 검사에서 홍채-수정체 가로막의 전방편위가 관찰되었다. 2일 뒤 좌안에도 같은 증상이 발생하였으며 구면렌즈대응치는 -15.5 D였다. 안저검사상 장액성망막박리와 형광안저촬영상 국소 노출이 동반되어 비전형적인 VKH병으로 진단하고 코르티코스테로이드를 경구투여하였다. 치료 시행 3주 뒤 증상들이 소실되었으며, 회복된 구면렌즈 대응치는 우안 -10.25 D, 좌안 -9.75 D였다. 결론: 비전형적인 VKH병의 초기 증상으로 안압 상승 없이 홍채-수정체 가로막의 전방 편위가 발생하여 근시가 진행할 수 있음을 경험하였다. 따라서 약물 복용력 없이 홍채-수정체 가로막의 전방 편위가 발생한 경우 VKH병의 동반 가능성을 염두에 두고 추적관찰을 해야 하겠다.

영문 초록

Purpose: We present a case of anterior displacement of the lens-iris diaphragm associated with Vogt-Koyanagi-Harada (VKH) disease. Case summary: A 35-year-old woman presented with decreased vision in her right eye. Examination revealed corrected visual acuity of 0.6 in the right eye and 1.0 in the left eye. Intraocular pressure (IOP) was 14 and 16 mmHg in the right and left eye, respectively. Spherical equivalent (SE) was -17.25 D in the right eye and -9.75 D in the left eye. Slit lamp examination and three-dimensional anterior segment photography revealed anterior displacement of the lens-iris diaphragm in both eyes. Two days later, the left eye developed similar symptoms with an SE of -15.5 D. Fundus examination revealed serous retinal detachment and fluorescein fundus photography demonstrated focal leakage leading to the diagnosis of atypical VKH disease. The patient was treated with oral corticosteroids and her symptoms resolved after 3 weeks. The Final SE was -10.25 D in the right eye and -9.75 D in the left eye. Conclusions: We suggest there is an association between atypical VKH and anterior lens-iris diaphragm displacement in the absence of elevated IOP; early myopic shift may be associated with this presentation. Therefore, patients with isolated lens-iris diaphragm displacement, particularly those without a history of medications should be monitored for the possibility of developing VKH disease.
