국문 초록

두개저는 그 해부학적 특성상 접근이 힘들어 이 부위에 발생한 종물의 외과적 절제는 고도의 수기를 요한다. 두개저의 외과적 접근은 병변의 침범 정도, 종물의 절제 가능성 여부 그리고 외과적 절제에 의한 치료 가능성등에 대한 정확한 평가가 중요하다고 할 수 있다. 저자들은 문헌에 보고되어 있는 다양한 두개저 접근법을 검토해 보았으며, 1990년도부터 1992년도 까지 전남대학교병원 구강악안면외과에서 두개저부에 발생한 양성 및 악성 병변으로 진단된 5 증례에서 두개저 접근 방법과 이에 따른 결과를 알아보았다. 잘 피낭된 외경막 양성병변의 경우 구내접근법을 이용하였으나, 재발된 양성종양이나 과도하게 침범한 악성종양의 경우는 응용된 이하선접근법과 안면골 절단술올 이용한 복합노출에 의해 술부의 광범위한 시야를 얻을 수 있었다. 이 증례들을 통해 두개저부 병변의 외과적 접근에 대해 다소의 지견을 얻었으나, 확실한 외과적 원리의 정립을 위해서는 좀 더 많은 수술경험례와 지속적인 관찰이 필요하리라 사료되었다.

영문 초록

Resection of a tumor located at the skull base and/or pterygomaxillary area requires an experienced operative technique because of limited surgical access and neighboring critical neurovascular strutures. The curability, possibility of complete mass excision and the degree of tumor invasion should be assessed prior to selecting any method of skull base access. In this paper, we report five representative cases of surgical access to the skull base which present the radiological work-up, operation and the postoperative results. Various surgical approaches combined with facial osteotomies were used according to the tumor location and its anatomical relationship. With the individualized surgical technique, a direct approach to the tumor with minimal morbidity was possible. Literature related to our cases were also reviewed.
