Evaluation of champy s method of fixating mandibular angle fractures in the in vitro model

국문 초록

하악골우각부골절시 Champy가 제안한 소형금속판 고정방법은 하악골상연에 하나의 소형금속판을 위치시켜 단피질나사로 고정하는 방법인데 이 방법은 구내접근법으로 금속판을 고정하기 때문에 이로 인한 여러가지 장점이 있다. 그러나 소형금속판을 위치시키는 장소인 하악골외사선의 모양이 매우 복잡해서 금속판을 하악골외사선의 모양에 정확히 맞추기가 어려우며, 이로 인하여 골절편에 비틀림작용을 미쳐 골절선의 하연을 벌어지게 만들 수 있다. 그리하여 본 연구에서는 하악골우각부골절시 사용되는 Champy고정방법의 기술적 문제점으로 야기되는 골절선의 이개량을 측정하였다. 10개의 사체하악골을 사용하여 각 하악골에서 양측 하악골우각부위에 골절단을 시행한 후 Champy가 제안한 방법으로 골절단을 정복고정한 후 골절편의 이개량을 골절편의 최하방점에서 캘리퍼를 사용하여 측정하였다. 골절편의 이개량은 1.4+0.8mm였다. 이러한 결과는 하악골우각부골절시 사용되는 Champy방법이 골절편의 하연에서 이개나 비틀림을 만들기 쉬운 방법임을 나타낸다.

영문 초록

Using 10 cadaver mandibles, amounts of technique-related cleavage of the fracture line in mandibular angle fractures were measured in the method of miniplate fixation proposed by Champy et al. It was found that the cleavage was 1.4±0.8mm at the inferior border of the fracture. The results of this study indicate that Champy s method is very susceptible to the tortional displacement or separation of the fractured fragments at the inferior border.
