여자 가출 청소년의 건강 행태 및 서비스 이용실태의 문제점과 지원방안 연구

국문 초록

본 연구는 여자 가출 청소년의 건강실태 및 서비스 이용현황을 파악하고 실효성 있는 지원 방안을 제시하고자 했다. 이를 위해 가출 경험 청소년 설문조사(205명) 및 전문가(24명) 의견조사를 실시하였다. 주요 조사결과는 첫째, 의식주 등 전반적인 생활실태가 열악하고 음주율 및 흡연율 등 건강위험 행동 역시 일반/여자 청소년들에 비해 높게 나타났다. 특히 쉼터에 입소해 있지 않은 경우, 불안정한 주거, 불규칙한 식사 및 수면, 건강지원 서비스 접근도에서 더욱 취약한 것으로 나타났다. 둘째, 성희롱, 성폭력, 성매매 등 폭력 상황에 노출되는 빈도가 높았으며, 성 관련 질환에서 취약함을 드러냈다. 셋째, 대상자들의 절반 정도는 의료지원 서비스를 받아본 적이 없는 것으로 나타났으며 서비스를 받은 경우 의료비 지원이나 병원 동행 등 사후적이고 단편적인 치료에 국한되어 있었다. 넷째, 대상자들은 건강에 대한 무관심, 병원에 대한 두려움과 거부감, 낙인으로 인한 진료 기피현상 등을 보였으며 이는 성장배경과 문화적 수치심 등 개인을 둘러싼 내외적 환경과 욕구의 문제와 관련되어 있었다. 이러한 연구결과를 토대로, 가출 여자 청소년의 건강 정책의 기본관점 및 접근방향, 지원 서비스의 내용, 접근성 제고 방안, 인프라 구축 방안 등을 제언하였다. 본 연구는 가출 여자 청소년의 건강문제를 본격화한 연구로서, 실태의 특성과 문제점을 파악하고 이용 접근성을 높일 수 있는 정책 개발을 위한 실천적 함의를 논의했다는 점에서 의의가 있다.

영문 초록

This study proposes effective support plan by understanding the health status and the current service usage situations of runaway teenage girls. To do so, the author has undergone surveys with 205 teenagers with runaway experiences as well as a pre-interview and conducted a Delphi survey on 24 professionals. The survey included elements such as individual traits, health and sexual activities, runaway life and health awareness, uses of health services and wants, and the understanding of the state of health and supports of runaway teenage girls by professionals. These factors were tested by analysis of frequency, cross tabulation analysis, Chi-square test, and independent samples T-test as well as one-way analysis of variance. Major research results indicated that the teenagers’ overall lifestyle was vulnerable due to an unstable residence and irregular meals and sleep. Moreover, experiences of sexual activities and the exposure rate of sexual diseases, pregnancy, rape, prostitution, smoking, alcohol consumption, and suicidal impulses showed higher rates compared to ‘regular’ girls. On the other hand, runaway teenagers who were not admitted into facilities showed symptoms of higher vulnerability in the health factors. Half of the surveyed teenagers have never received any health services and showed that medical examination of obstetrics and gynecology and psychiatry to be the most ‘humiliating’ and the most needed medical services, which demonstrated that there needs to be an increase in awareness as well as accessibility. However, according to the surveys conducted on professionals, the results suggested that current service system is heavily concentrated on the teenagers that are admitted into a facility and limited to ex-post medical services, and therefore needs a more integrated and precautionary measure. As this study deals with the health problems of runaway teenage girls, it is significant in aspects of attempting to discuss practical implications of developing health support related policies in order to improve accessibility for both admitted and non-admitted runaway teenagers by differentiating the two groups and analyze their current status.
