간호대학생의 좋은 죽음인식과 연명의료중단 태도와의 관계에서 사전연명의료의향서 지식의 매개 효과

국문 초록

목적 본 연구는 간호대학생의 좋은 죽음인식, 사전연명의료의향서 지식, 연명의료중단 태도와의 관계를 확인하고 좋은 죽음인식과 연명의료중단 태도의 관계에서 사전연명의료의향서 지식의 매개효과를 확인하기 위함이다. 방법 연구 대상자는 경기도 소재 C 대학과 경상남도 소재 G 대학의 간호대학생 167명을 대상으로 좋은 죽음인식, 사전연명의료의향서 지식, 연명의료중단 태도를 온라인 설문지를 이용하여 자료를 수집하였다. 자료 분석은 Spearman 상관 분석, Baron과 Kenny의 3단계 매개회귀 분석 및 Sobel test, Hayes의 Process macro (model 4)로 분석하였다. 결과 좋은 죽음인식이 높을수록 사전연명의료의향서 지식이 높았고(r=.180, p=.020) 연명의료중단 태도가 높아지는 관계를 보였다(r=.213, p=.006). 사전연명의료의향서 지식이 높을수록 연명의료중단 태도가 높아지는 양의 상관관계를 보였다(r=.247, p<.001). 좋은 죽음인식은 사전연명의료의향서 지식과 연명의료중단 태도를 높이는 영향 요인이었으며(B=0.22, p=.015; B=0.03, p=.009) 좋은 죽음인식과 연명의료중단 태도와의 관계에서 사전연명의료의향서 지식의 매개효과를 확인하였다(B=.039, LLCI=.006, ULCI=.084). 결론 간호대학생의 연명의료중단 태도를 향상시키기 위해 좋은 죽음인식과 사전연명의료의향서 지식을 높일 수 있는 교육프로그램 개발을 위한 자료로 활용할 수 있을 것이다.

영문 초록

Objectives This study examines the mediating effect of knowledge of advance directives in the relationship between good death perception and attitude toward withdrawal of life-sustaining treatment among nursing students. Methods In this study correlational study, 167 nursing students from two nursing colleges completed questionnaires of a good death perception, a knowledge toward advance directives, and an attitude toward withdrawal of life-sustaining treatment. Data were analyzed Spearman correlational coefficients, three-step regression analysis, and Hayes Process Macro analysis. Results Good death perception was correlated with the knowledge of advance directives (r=.180, p=.020) and the attitude of withdrawing life-sustaining treatment (r=.213, p=.006). Good death perception influenced the knowledge of advance directives and attitude toward withdrawal of life-sustaining treatment (B=0.22, p=.015; B=0.03, p=.009). Knowledge of advance directives mediated the relationship between Good death perception and attitude of withdrawing life-sustaining treatment (B=.039, LLCI=.006, ULCI=.084). Conclusions Learning strategies aiming to enhance attitude toward withdrawal of life-sustaining treatment require successful good death perception to boost knowledge of advance directives among nursing students.
