엎드린 자세에서 Ferguson view 검사의 유용성 평가

국문 초록

목 적 : Ferguson view 검사에서 환자의 자세와 X선 입사각도에 따른 영상과 유효선량을 각각 비교하여 엎드린 자세에서 Ferguson view 검사의 유용성을 입증하고자 한다. 대상 및 방법 : 인체모형팬텀을 사용하여 바로 누운 자세에서 머리 쪽으로 각각 20 o , 25 o , 30 o , 35 o , 40 o 엎드린 자세에서 다리 쪽으로 각각 20 o , 25 o , 30 o , 35 o , 40 o 각도로 입사하여 영상을 획득하였다. PCXMC 프로그램을 사용하여 유효선량을 산출하였으며 PACS의 길이 측정용 Tool을 이용하여 5번째 요추와 천추사이의 거리를 측정하였다. 측정을 통해 얻어진 자료는 대응 t검정으로 유의한 차이를 분석하였다. 결 과 : 엎드린 자세에서 다리 쪽으로 30 o X선을 입사하였을 때 평균 8.39±0.13으로 5번째 요추와 천추사이 공간이 가장 넓었고, 엎드린 자세에서 다리 쪽으로 35 o 입사하였을 때 평균 1.76±0.11 로 가장 좁게 나타났다. 누운 자세에 따른 5번째 요추와 천추사이 공간은 통계적 차이가 있었다(P<0.05). DAP값과 유효선량은 누운 자세에 따라 입사방향을 머리 쪽과 다리 쪽으로 30 o 입사하였을 때 각각 88%, 42%로 가장 큰 차이가 있었으며 20 o 로 입사하였을 때 각각 97%, 65%로 가장 적은 차이를 보였다. 누운 자세에 따른 DAP값과 유효선량은 각각 통계적 차이가 있었다(P<0.05). 결 론 : 수술 자세와 동일한 엎드린 자세에서의 Ferguson view 검사는 영상의 질을 유지 하면서도 유효선량을 감소시킬 수 있었으며, 임상 적용 시 진단적 가치와 환자의 만족도를 높일 수 있으리라 사료된다.

영문 초록

Purpose : Proved the usefulness of the Ferguson view in the prone position compare with the image quality and the effective dose about the patient position and the X-ray incident angle. Materials and Methods : Exposure for the cranial 20 o , 25 o , 30 o , 35 o , 40 o X-ray towards the phantom in the supine position and the caudal 20 o , 25 o , 30 o , 35 o , 40 o in the prone position. Then we get the images of all parameters. Calculate the effective dose using the PCXMC program and the measure the l-s joint space length using the tool of PACS. We analyze the difference of measured data by the t-test. Result : The prone-caudal 30 o s lumbo-sacral joint space length is the longest in supine and prone position(8.39±0.13). and prone-caudal 35 o s lumbo-sacral joint space length is the shortest in supine and prone position(1.76±0.11). The l-s joint space distance has the statistical difference in the patient position(p<0.05).The DAP value and effective dose has the statistical difference in patient position(p<0.05). The biggest difference about the DAP and effective dose is the 30 o in both position and incident angle at the 88%, 42%. The smallest difference is the 20 o at 97%, 65%. Conclusion : Ferguson view in the same position with the surgery is maintain the suitable image quality and reduce the effective dose. Also it is develop the diagnostic value in clinic and the satisfied of patient.
