외상성 뇌손상 환자에게 적용한 인지 중재 방법에 대한 체계적 고찰

국문 초록

목적 본 연구에서는 외상성 뇌손상 환자에게 적용한 인지 중재의 접근법에 대한 문헌 고찰을 통해 외상성 뇌손상 환자에게 적용한 인지 중재에 대한 효과를 비교 분석하고 효과적인 중재 방법을 제시하고자 한다. 연구방법 2015년부터 2025년까지의 전자 데이터 베이스인 Riss, DBpia, KISS, Pubmed, Google Scholar를 사용하였고, 주요 검색 용어로 ‘인지’, ‘외상성 뇌손상’, ‘중재’, ‘cognition’, ‘intervention’, ‘TBI’, ‘traumatic brain injury’를 사용하였다. 배제기준을 통해 최종적으로 7개의 연구를 분석대상으로 선정하였다. 결과 인지 중재는 외상성 뇌손상 환자의 주의력, 기억력, 실행 기능 등 다양한 인지 영역 개선에 긍정적인 효과를 나타냈다. 특히, 집중력 강화와 일상생활 수행 능력 향상에 유의미한 변화를 보인 연구들이 다수 확인되었다. 그러나 연구 설계의 이질성 및 표본 수의 한계로 인해 일관된 결론을 도출하는 데에는 일부 제한점이 존재하였다. 결론 본 체계적 고찰을 통해 외상성 뇌손상 환자에게 인지 중재가 인지 기능 향상에 긍정적인 영향을 미친다는 근거를 확인할 수 있었다. 향후에는 표준화된 연구 설계와 대규모 표본을 활용한 추가 연구가 필요하며, 다양한 중재 기법 간의 비교 연구 또한 요구된다.

영문 초록

Objective This study aims to propose effective intervention strategies that reflect behavioral changes in patients with traumatic brain injury by systematically reviewing the effectiveness of cognitive intervention therapy applied to this population. Methods Electronic databases Riss, DBpia, KISS, Pubmed, and Google Scholar were used from 2015 to 2025, and the main search terms were ‘traumatic brain injury’, ‘cognition’, and ‘intervention’. Through the exclusion criteria, 7 studies were finally selected for analysis. Results The analysis showed that cognitive mediation therapy had a positive effect on improving various cognitive domains, including attention, memory, and executive function in patients with traumatic brain injury. In particular, many studies were found to have shown significant changes in improving concentration and the ability to carry out daily activities. However, there were some limitations in drawing consistent conclusions due to the heterogeneity of study designs and limited sample sizes. Conclusion This systematic review confirmed the evidence that cognitive mediation therapy has a positive effect on improving cognitive function in patients with traumatic brain injury. Further studies using more standardized research designs and large samples are needed, as well as comparative studies between various mediation techniques.
