Objective: This study compared the activity of accessory respiratory muscles namely the upper trapezius, — sternocleidomastoid, and external oblique abdominis, during inhalation resistance exercise in children with spastic cerebral palsy. Design: Acute Pre-Post DesignMethods: Seven children with spasticity and cerebral palsyparticipated in this study. Surface electromyography was used to assessthe activity of the upper trapezius, sternocleidomastoid, and external abdominal oblique muscles. The POWERbreathePlus devicewas used to apply intake resistance in all postures. Results: A significant difference in muscle activity was observed in the supine position in the upper trapezius (10.86 ± 3.71),sternocleidomastoid (11.33 ± 3.36), and external oblique (6.09 ± 5.02), and in the sitting position in the upper trapezius (12.85 ±3.89), sternocleidomastoid (6.34 ± 1.82), and external oblique (4.02 ±1.4). Conclusions: This study confirmed that inspiratory resistance training increased the activity of accessory respiratory muscles, including the upper trapezius, sternocleidomastoid, and external oblique, in children with spastic cerebral palsy in both the supineand sitting positions. These findings suggest that such training may enhance respiratory muscle function, improve breathingefficiency, and potentially supportbetter posture and motor control in this population. This study provides exploratory evidence forincorporating inspiratory resistance training into rehabilitation programs aimed at improving respiratory and functional outcomesin children with spastic cerebral palsy.
