국문 초록
Background The strength of the upper trapezius is essential for effective scapular elevation, upward rotation, and overall shoulder stability. The standard strength assessment position—shoulder shrug (SP)—may lead to compensatory activation of the levator scapulae in individuals with upper trapezius weakness, thereby limiting the accuracy of the evaluation.
Objectives The purpose of this study was to determine the most suitable posture for assessing upper trapezius strength through a comparison of muscle activity between the upper trapezius and the levator scapulae.
Methods Thirty-seven healthy adults in their 20s to 30s participated in the study. The strength and surface electromyographic (sEMG) activity of the upper trapezius and levator scapulae were measured at three shoulder abduction angles: 0°, 140° (in the scapular plane), and 180°. A one-way repeated measures ANOVA (α = 0.05), with Bonferroni correction (α = 0.016), was conducted to compare UT, LS, and UT/LS ratio in muscle activity and strength across three shoulder abduction angles (0°, 140°, 180°). Reliability was evaluated using intraclass correlation coefficients (ICCs).
Results Upper trapezius strength at 0° was significantly greater than at 140° and 180° (p < 0.05). The UT/LS activity ratio was significantly lower at 0° compared to 140° and 180° (p < 0.001). Excellent reliability (ICC ≥ 0.90) was observed for both test-retest and inter-rater measurements at 140° and 180°, whereas 0° showed lower reliability (ICC = 0.86 and 0.77, respectively).
Conclusions The 140° shoulder abduction position demonstrated the highest UT/LS activity ratio and excellent reliability, indicating that it may serve as a more effective alternative to the standard shoulder shrug position for accurate upper trapezius strength assessment in clinical settings.
