암 생존자의 동반질환 수준이 우울에 미치는 영향: 건강관리 준비 상태의 매개효과

국문 초록

본 연구는 암 생존자의 동반질환 수준이 우울에 미치는 영향을 확인하고, 동반질환 수준과 우울의 관계에서 건강관리 준비 상태의 매개효과를 분석하는 것을 목적으로 하였다. 본 연구는 <한국형 암 생존자를 위한 건강관리 평가도구 개발 연구>의 원자료를 활용한 2차 자료 분석 연구로, 수도권 소재 병원에서 유의추출법을 통해 모집된 유방암 및 대장암 생존자 173명을 대상으로 하였다. 우울은 단축형 간이진단검사-18(BSI-18)의 우울 하위척도를 사용하여 측정하였으며, 건강관리 준비 상태는 건강관리 준비 척도(PTSCI)를 활용하여 평가하였다. 연구 결과, 암 생존자의 동반질환이 많을수록 건강관리 준비 상태가 낮고, 우울 수준은 높은 것으로 나타났다. 또한, 건강관리 준비 상태는 동반질환과 우울 간의 관계를 완전히 매개하는 것으로 나타나, 건강관리 준비 상태가 우울 감소에 있어 중요한 역할을 하고 있음을 확인하였다. 이러한 연구 결과는 의료현장에서 암 생존자의 심리·사회적 문제에 효과적으로 개입하기 위해, 동반질환과 건강관리 준비 상태를 동시에 고려하는 통합적 접근의 필요성을 시사한다. 나아가, 건강관리 준비 상태를 정기적으로 평가하고, 이를 향상시키기 위한 교육 및 개입 전략이 병행되어야 함을 강조하였다.

영문 초록

This study aimed to examine the impact of comorbidity levels on depression among cancer survivors and to analyze the mediating effect of preparedness for transition to survivorship care in the relationship between comorbidity and depression. A secondary data analysis was conducted using the original dataset from the project titled “Development of a preparedness for transition to survivorship care Tool for Korean cancer survivors.” A total of 173 breast and colorectal cancer survivors were recruited through purposive sampling from public tertiary hospitals in the metropolitan area. Depression was measured using the depression subscale of the Brief Symptom Inventory-18 (BSI-18), and preparedness for survivorship care transition was assessed using the Preparedness for Transition to Survivorship Care Instrument (PTSCI). The results indicated that survivors with higher levels of comorbidity exhibited lower levels of preparedness for survivorship care transition and higher levels of depression. Furthermore, preparedness for survivorship care transition fully mediated the relationship between comorbidity and depression, highlighting its critical role in mitigating depressive symptoms. These findings underscore the necessity of an integrated approach that simultaneously considers comorbidity and preparedness when addressing the psychosocial needs of cancer survivors in clinical settings. Regular assessments of preparedness, coupled with targeted educational and intervention strategies to enhance it, are strongly recommended.
