국문 초록

중절치 결손과 구순 돌출이 있는 중년 환자의 경우 치료에 앞서 정밀한 치료계획의 수립이 필요하다. 돌출 해소를 위해 일반적으로 고려하는 소구치 발치를 진행할지, 결손된 중절치 공간을 측절치 및 견치 대체로 폐쇄할지에 대한 판단은 필요한 치아 이동량, 환자의 치주 및 치아 상태를 고려한 치료 결과의 예후, 전치 대체 시의 미소 심미 등을 복합적으로 고려해야 한다. 본 증례는 상악 중절치와 하악 측절치, 상악 제2대구치가 결손 또는 발거 예정이었던 중년의 환자에서 자연치를 최대한 보존하는 치료 계획을 수립하여, 상악 측절치를 중절치로, 견치를 측절치로 대체하여 양호한 교합 및 안모와 미시 심미를 개선을 얻었기에 이를 보고하고자 한다.

영문 초록

Multiple premolars are frequently extraction to improve adult Class II malocclusion with protrusion. However, when multi-ple missing, hopeless teeth, edentulous sites or bridge resto-rations are pre-existing, customized extraction schemes may be necessary especially in aged adults. Determining whether to extract the maxillary premolars, or to close the edentulous space or the restorated site depends on the amount of tooth movement required, the prognosis considering the patient’s periodontal and dental conditions, and the overall dental es-thetics when teeth substitution is planned. This case illustrates how the bilateral pontic space of the maxillary central incisors was substituted with lateral incisors and laterals with canines as well as the hopeless second maxillary molar with the third molar instead of premolar extraction to improve protrusion in a middle-aged adult.
