선수술 후 안정성에 관한 고찰

국문 초록

선수술 후 턱뼈의 위치 안정성, 즉 회귀 가능성에 대하여는 다양한 의견이 제시되었다. 본 연구에서는 2010년부터 11년간 선수술-후교정의 방법으로 진료를 받았던 91명 중 술직후와 장기적 안정성 측면에서 모두 부정적인 반응을 보였던 6명을 확인하고, 이들의 선수술 후 골격 및 치아의 불안정성을 조사하고 이와 연관된 인자들을 확인하고자 하였다. 1년 이상 장기 관찰 자료가 미비한 1명을 제외한 5명의 증례에 대하여 3차원 분석과 치료 경과를 검토하고, 수술 술식을 비교하였으며, 술전으로부터 술후 약 1년까지의 영상 자료를 이용한 2차원 분석 및 3차원 모델 비교 분석을 수행하여 골조직-치아 변화 패턴을 확인하였다. 5증례의 분석 결과 수술 후 회귀성 하악골 전방 이동은 2증례에서만 발생하고 나머지는 일반적 술식(IVRO)에서와 같은 후방 이동 결과를 보였다. 또 치아의 이동 방향이 하악골 이동 방향과 일치하지 않아서, 술후 교합 불안정 혹은 개교증 발생이 하악골 회귀와 연결되지 않았다. 또 모든 증례들에서 수술 전 개교증이나 혀를 밀고 있는 영상이 관찰되었다. 이들 결과들을 종합하면 하악골의 위치 변화와 상관 없이 혀의 미는 힘에 따른 상하악 전치의 위치-각도 변화가 발생하여 수술 후 치아-교합 불안정성이 발생한 것으로 생각할 수 있었다. 따라서 혀의 기능적 작용이 선수술 후 골격-치아-교합 안정성 유지의 주요 변수 중 하나가 될 수 있겠다. 물론 혀의 기능 외에도 저작근의 작용과 골치유 및 리모델링, 턱관절, 등도 영향을 미칠 수 있기에 이들을 함께 고려하는 추가 연구가 필요할 것이다.

영문 초록

There have been various reports on postoperative skeletal stability and relapse using the surgery-first approach. From the 91 patients who were treated using the surgery-first approach between 2010 and 2020, six individuals with poor short- and long-term stability were selected for this study. Skeletal and dental changes were analyzed to identify factors related to postoperative stability. One patient was excluded because of the absence of long-term follow-up data. The three-dimensional analysis and treatment progress of the remaining five cases were evaluated, and the surgical techniques were compared. Furthermore, with the help of radiological data obtained preoperatively up to 1 year postoperatively, two-dimensional and three-dimensional analyses were performed to examine skeletal and dental changes. Analysis of five cases showed that recurrent anterior movement of the mandible occurred in only two cases. The remaining cases showed posterior movement, as was also observed in patients treated with the conventional approach (IVRO). Furthermore, postsurgical instability in occlusion or open bite was not related to postsurgical relapse of the mandible since the direction of tooth movement did not coincide with the direction of mandibular movement. Additionally, in all cases, open bite or tongue thrusting was observed on the presurgical images. In conclusion, regardless of the change in the mandibular position, the force generated by tongue thrusting affects the inclination of the maxillary and mandibular incisors, leading to postsurgical instability in the position of the teeth and occlusion. Accordingly, the functional aspects of the tongue can serve as one of the important factors that influence the skeletal and dental stability after the surgery-first approach. Follow-up studies are needed to determine the influence of additional factors, such as the masticatory muscles, bone healing and remodeling, and temporomandibular joints, on postsurgical stability.
