The Impact of Data Breaches on Hospital Insurance Spending

국문 초록

이 연구는 미국 병원에서 발생한 사이버 데이터 유출 사고가 병원의 보험 지출에 미치는 영향을 실증적으로 분석하였다. 2017년부터 2019년까지의 병원 연도별 데이터를 활용하여, American Hospital Association(AHA) 연례조사, CMS 병원 재무보고(HCRIS), 미국 보건복지부(HHS)의 Breach Portal 데이터를 병합하였다. 본 연구의 주요 독립변수는 해당 연도에 데이터 유출 사고를 경험한 병원 여부이며, 종속변수는 병원의 연간 보험 지출로 설정하였다. 성향점수매칭(Propensity Score Matching)기법과 다변량 회귀분석을 통해 교란변수를 통제하고 평균처리효과(ATE)를 추정하였다. 기술통계 결과, 데이터 유출을 경험한 병원은 일반적으로 규모가 크고 재정적으로 안정적이며 복잡한 환자군을 보유하고 있었다. 하지만 통제변수를 적용한 분석에서는 데이터 유출 사고와 보험 지출 간에 통계적으로 유의한 관계는 관찰되지 않았다. 이는 병원이 보험료 인상 등 외부적 조치보다는 내부 보안 투자 또는 자가 보험(self-insurance)전략을 활용하고 있음을 시사한다. 아울러 보험료는 보통 계약 갱신 주기에 따라 조정되므로, 사고 직후의 보험 지출 변화는 지연될 수 있음을 암시한다. 따라서 보험 지출만으로는 데이터 유출 사고의 재정적 영향을 충분히 설명할 수 없으며, 보다 폭넓은 사이버 리스크 대응 전략이 필요하다.

영문 초록

This study investigates whether data breaches lead to measurable changes in hospital insurance expenditures in the U.S. healthcare sector. Using a panel dataset of hospital-year observations from 2017 to 2019, we constructed a binary indicator for data breach exposure and examined its association with reported insurance expenditures. To control for institutional differences, we applied propensity score matching (PSM) and estimated the average treatment effect on treated using a multivariate regression model. Descriptive statistics revealed that breached hospitals were generally larger, more complex, and financially robust institutions. However, after adjusting for observed characteristics, we found no statistically significant association between data breaches and changes in insurance expenditures. This suggests that hospitals may not experience immediate insurance premium increases or may manage breach-related risks through self-insurance strategies such as self-insurance.
