선행 질병력의 의료비 자료 분석을 통한 중대질병 발생 예측모형에 관한 연구

국문 초록

고령화와 저출산 문제로 의료비 부담은 우리 사회의 큰 문제 중 하나로 부각되고 있다. 최근 의료의 패러다임이 치료중심에서 질병예방과 건강관리에 초점을 맞추는 방향으로 전환되고 있는 상황에서 건강관리 서비스의 기반이 되는 다양한 질병의 예측모형들을 제시하는 논의와 관련 연구가 활발해지고 있다. 본 연구에서는 의료비 지출이 많은 주요 질병들을 중대질병으로 정의하고, 중대질병에 영향을 미치는 유의한 선행질병을 파악하고 해당 선행질병의 의료비 정보를 바탕으로 중대질병의 발생 위험도를 평가하는 모형을 제시하였다. 국민건강보험공단의 코호트 DB를 바탕으로 로지스틱 회귀모형과 주성분분석을 통해 중대질병 발생의 예측 모형을 도출하였다. 또한, 해당 모형을 바탕으로 중대질병의 위험도를 나타내는 평점 산출방법과 중대질병의 위험도에 따라 집단을 구분할 수 있는 등급 체계를 제시하였다. 개인의 의료비 정보와 중대질병에 대한 선행질병의 영향도를 활용하여 중대질병을 예방하고 건강한 삶을 영위할 수 있는 건강관련 서비스의 제공의 토대를 마련하는 데 기여할 수 있을 것으로 기대된다.

영문 초록

The burden of medical expenses has become one of the major issues in our society due to the problems of aging and low birth rates. Recently, as the healthcare paradigm shifts from a treatment-centered approach to one focused on disease prevention and health management, discussions and research on various predictive models for diseases that form the foundation of health management services have become more active. This study defines major diseases, which incur high medical costs, as serious diseases, identifies preceding diseases that significantly affect the occurrence of serious diseases, and proposes a model to assess the risk of serious disease occurrence based on the medical cost information of these preceding diseases. Using the cohort database of the National Health Insurance Service, a predictive model for serious disease occurrence was derived through logistic regression and principal component analysis. Additionally, based on this model, a scoring method to represent the risk of serious diseases and a grading system to categorize groups based on their risk levels were proposed. It is expected that utilizing individual medical cost information and the impact of preceding diseases on serious diseases will contribute to the prevention of serious diseases and provide a foundation for offering health-related services that help individuals lead healthy lives.
