전신 알레르기 질환과 브리모니딘 점안으로 인한 알레르기결막염의 관계

국문 초록

목적: 녹내장환자에서 안압을 하강시키기 위한 약물로 브리모니딘(Brimonidine tartrate 0.1%)은 흔하게 사용되는 안약으로 종종 알레르기결막염을 부작용으로 동반한다. 본 연구에서는 브리모니딘 사용환자에서 발생할 수 있는 알레르기결막염의 정도와 전신 알레르기 질환과의 상관관계를 비교 분석하고자 한다. 대상과 방법: 본 연구는 임상 증례 연구로서 연구 대상자는 원발개방각녹내장 또는 정상안압녹내장으로 진단받은 환자 중 브리모니딘사용으로 알레르기결막염이 발생한 환자 63명(63안)으로 구성되었다. 알레르기결막염이 새롭게 발생한 환자들을 대상으로 천식, 알레르기비염, 두드러기 증상에 대한 설문조사를 시행하였고, 세극등현미경검사를 통해 알레르기결막염의 정도를 외래에서 측정하여 관련성을 비교 분석하였다. 결과: 천식, 알레르기비염, 두드러기환자는 각각 4명, 18명, 6명이었고, acute conjunctivitis disease score는 각각 11-22점, 1-211점, 12-100점으로 다양하였다. 천식과 알레르기결막염의 정도는 통계적으로 유의한 관계를 보이지 않았다(r=-0.095, p=0.461). 또한 알레르기비염(r=0.117, p=0.361)과 두드러기(r=-0.110, p=0.389) 역시 알레르기결막염의 정도와 통계적으로 유의한 관계를 보이지 않았다. 결론: 본 연구 결과는 녹내장환자에게서 전신 알레르기 질환이 브리모니딘 점안 시 발생할 수 있는 알레르기결막염의 정도와 관련이 있지 않다는 것을 보여주었다. 이는 전신 알레르기 질환이 있는 환자군에게서 브리모니딘 점안이 필요할 때, 보다 폭넓게 안약을 사용 할 수 있음을 시사한다. <대한안과학회지 2019;60(3):233-238>

영문 초록

Purpose: Several different topical eye drop medications are used to lower the intraocular pressure of glaucoma patients. Among hem, 0.1% brimonidine tartrate is commonly used and accompanies allergic conjunctivitis as a side effect. This study determined the correlation between the severity of allergic conjunctivitis arising from brimonidine eye drop use and systemic allergic disease. Methods: A case report study of 63 selected patients diagnosed with open-angle glaucoma or normal-tension glaucoma, and developing allergic conjunctivitis due to brimonidine usage was conducted. The study surveyed patients with newly diagnosed allergic conjunctivitis in terms of allergic rhinitis, asthma, and urticaria symptoms, and a slit lamp examination was used to determine the severity of allergic conjunctivitis in the clinic, using a novel scoring system to establish correlations of interest. Results: There was no correlation between asthma and the severity of allergic conjunctivitis (r = -0.095, p = 0.461). Allergic rhinitis showed no significant correlation with the severity of allergic conjunctivitis (r = 0.117, p = 0.361), and urticaria displayed no significant correlation with allergic conjunctivitis (r = -0.110, p = 0.389). We found no correlation in glaucoma patients between brimonidine eye drop use and the severity of allergic conjunctivitis. Conclusions: Systemic allergic disease was not related to the severity of allergic conjunctivitis arising from the use of brimonidine eye drops, indicating that the use of these eye drops can effectively lower intra-ocular pressure even if the patient suffers from systemic allergic disease.
