열사병 이후 발생한 운동 신경원 질환 1예

국문 초록

열사병은 심부 체온의 상승(섭씨 40도 이상)과 중추 신경계 이상에 의한 의식 장애로 특징지어지는 내과적인 응급질환 중의 하나이다. 열사병 이후 동반되는 신경학적 이상은 흔히 소뇌, 기저핵, 말초신경, 척수 등의 손상과 연관되는데, 저자들은 열사병 이후 말초 신경의 손상 없이 운동 신경원성 질환이 발생한 환자를 근전도를 통해 진단하였기에 이에 대해 보고하고자 한다. 몇 겹의 속옷을 입은 채로 전기 장판에 누워 있던 60세 남자 환자가 내원하였고, 당시 환자의 의식은 기면 상태였으며 심부 체온은 섭씨 41도였다. 고열과 의식 장애에서 회복된 후 감각의 이상이 없는 사지마비 소견이 관찰되어 뇌, 경추, 흉추, 요추 부위 자기 공명 영상 검사를 시행하였으나 특이 소견이 관찰되지 않았다. 뇌 양전자 단층 촬영에서 양측 소뇌를 포함하여 양측 뇌에 동일하게 포도당 대사가 저하되어 있는 소견이 관찰되었고, 신경 전도 검사와 침근전도 검사에서 운동 신경원성 질환에 합당한 결과가 확인되었다. 이에 저자들은 환자의 감각 손상 없는 사지마비가 열사병 이후 고열에 의해 발생한 뇌 손상 및 운동 신경원 세포의 손상에 의한 것이라고 결론을 내릴 수 있었다.

영문 초록

Heat stroke is a medical emergency, with the two cardinal features being raised core body temperature(＞40℃) and central nervous system dysfunction. Most cases of neurologic deficit after heatstroke are associated with cerebellum, basal ganglia, peripheral nerve, spinal cord. But, we describe a case of motor neuron disease without peripheral nerve injury after heatstroke. A 60-years-old male was found in his bed on an electric blanket wrapped him up with several under-wears. When admitted, his mental state was drowsy and body temperature was above 41℃. After regained consciousness, quadriplegia became apparent without other sensory deficits. No abnormality was observed on Brain, cervical, thoracic and lumbo-sacral magnetic resonance imaging. But brain positron emission tomography shows mild symmetric hypo-metabolism in the brain, especially bilateral cerebellum. Nerve conduction study and needle electromyography are suggestive of motor neuron disease. We conclude his irreversible neurologic deficits induced by brain hypo-metabolism and thermal insult on motor neuron. In addition, Impaired thermoregulation due to alprazolam, fluoxetine can be responsible for the development of heat stroke.
