Erysipelas is a superficial bacterial skin infection that typically affects the lower limbs and face. However, involvement of the auricle is rare and may be overlooked due to its uncommon presentation and potential for misdiagnosis. We present a case of a 61-year-old female who developed sudden erythema, swelling, and tenderness of the left auricle following manual rubbing of the ear. Milian’s ear sign—erythema involving both the cartilaginous and lobular parts of the auricle—was noted, supporting the diagnosis of auricular erysipelas. Laboratory findings revealed mild leukocytosis and elevated C-reactive protein. The patient was treated with intravenous vancomycin and ceftazidime, resulting in rapid clinical improvement and complete resolution within eight days. Auricular erysipelas is clinically significant because of the anatomical features of the pinna and its susceptibility to rapid spread and complications such as perichondritis. Differentiating it from other conditions— including cellulitis, relapsing polychondritis, and herpes zoster oticus—is critical. In addition to systemic antibiotic therapy, adjunctive topical skin care and patient education to avoid trauma are essential, especially given the risk of recurrence. This case highlights the diagnostic value of Milian’s ear sign and the importance of timely intervention to prevent long-term complications.
