국문 초록

이 연구는 건강 관련 삶의 질을 측정하는 EQ-5D 지표의 분포에서 나타나는 천장효과, 데이터의 높은좌측 왜도 및 음수값 포함과 같은 비정규적 특성을 고려하여 베이지안 기반 two-part 모형을 적용하였다. 1단계에서는 완전건강 여부를 종속변수로 하는 이항 로지스틱 회귀분석을 수행하였으며, 2단계에서는비건강집단의 EQ-5D 값을 beta 정규화와 logit 변환 후 Gaussian 기반 일반화선형혼합모형으로 분석하였다. EQ-5D 분포에 영향을 미치는 다양한 요인을 실증적으로 검증하기 위해 2019년 지역사회건강조사의원시자료 내 202,490명을 활용하였다. 분석 결과, 도시 거주, 고학력, 고소득일수록 완전건강 상태일가능성이 높았으며, 비완전건강 집단 내에서도 높은 건강 수준을 유지하였다. 반면, 주관적 스트레스수준은 두 단계 모두에서 건강 상태에 부정적 영향을 미친 반면, 사회적 관계 및 비형식적 사회참여(친목활동, 여가활동)는 특히 비완전건강 집단 내 건강 수준에 긍정적인 영향을 주었다. 본 연구는 기존의연구들이 간과한 EQ-5D의 분포적 특성을 고려하여 건강 상태의 이분성과 연속성을 동시에 분석하였으며, 건강정책 수립 시 건강 수준의 연속적 다양성을 반영한 정교한 접근이 필요함을 강조하였다.

영문 초록

This study employs a Bayesian two-part model to address the non-standard distributional characteristics of the EQ-5D, a widely used measure of health-related quality of life. It specifically focuses on challenges such as ceiling effects, pronounced left skewness, and the presence of negative values. The two-stage framework consists of a binary logistic regression to model full health status and a Gaussian mixed model applied to beta-normalized and logit-transformed EQ-5D scores for individuals reporting less than full health. Using data from 202,490 respondents in the 2019 Korea Community Health Survey, the results reveal that urban residency, higher educational attainment, and higher income levels are positively associated with better health outcomes. In contrast, subjective stress adversely affects health status, whereas informal social participation positively influences it, particularly among individuals in less than full health. These findings underline the necessity for targeted and nuanced health policies that acknowledge continuous variations in health status and account for diverse demographic and socioeconomic determinants.
