국문 초록

정신질환은 만성적인 경과를 보이는 경우가 많아, 지역사회에서 지속적인 회복과 적응이 중요하며, 증상 중심의 의료적 접근을 넘어 정신질환자가 자율성과 주체성을 갖고 살아가기 위해 '재기(recovery)'의 개념이 강조되고 있다. 본 연구는 지역사회 거주 정신질환자의 재기태도에 영향을 미치는 요인을 파악하고, 효과적인 간호중재 개발을 위한 기초자료를 제공하고자 하였다. 지역사회에 거주하는 정신질환자를 대상으로 한 서술적 조사연구로서, 부산광역시 소재 정신보건시설 이용자 140명을 대상으로 내재화된 낙인, 자아존중감, 사회적 지지, 재기태도는 표준화된 도구를 사용하여 측정되었다. 다중회귀분석 결과, 재기태도는 내재화된 낙인과는 부적 상관관계, 자아존중감과 사회적 지지와는 정적 상관관계를 보였으며, 진단명과 거주형태 또한 유의한 영향요인으로 확인되었다(설명력 42.4%). 진단별로는 조현병의 경우 내재화된 낙인, 양극성 장애는 자아존중감, 우울장애는 사회적 지지가 주요 핵심 영향요인으로 나타났다. 이러한 결과는 정신질환자의 재기를 위해 낙인을 줄이고, 내적‧외적 자원을 강화하는 맞춤형 간호중재가 필요함을 시사한다.

영문 초록

Mental illness often follows a chronic course, highlighting the need for sustained recovery and adaptation in the community. Emphasizing autonomy beyond symptom-based care, this study identified factors influencing recovery attitudes to inform nursing intervention development. A descriptive survey was conducted with 140 community-dwelling individuals using mental health facilities in Busan. Standardized tools measured diagnosis, residential status, internalized stigma, self-esteem, social support, and recovery attitude. Multiple regression showed that internalized stigma had a negative effect, while self-esteem and social support had positive effects on recovery attitude. Diagnosis and residential status were also significant predictors (R² = .424). By diagnosis, schizophrenia was most affected by stigma, bipolar disorder by self-esteem, and depression by social support. These findings suggest the need for tailored nursing interventions that reduce stigma and enhance personal and social resources.
