청소년의 흡연 경험 유무와 정신건강 지표와의 관련성

국문 초록

본 연구는 청소년의 흡연 경험 유무와 중학생과 고등학생의 정신건강 상태의 차이를 규명하고자 하였다. 본 연구는 2022년 청소년건강행태조사 데이터를 활용한 이차 분석 연구로 수행되었다. 연구대상은 중학생 26,182명 및 고등학생 22,484명 등 총 48,666명이었으며, 이 자료에 대하여 교차분석과 다중회귀분석을 실시하였다. 연구 결과, 흡연 경험이 있는 청소년은 비흡연 청소년에 비해 우울감, 외로움, 수면 부족, 자살 생각 및 자살 시도율이 유의하게 높았다(자살 시도율: 흡연군 6.9%, 비흡연군 1.6%; p<.001). 또한 고등학생이 중학생보다 전반적으로 이러한 정신건강 위험군에 더 많이 포함되었다. 다중회귀분석 결과, 청소년의 흡연 행태에 가장 큰 영향을 미치는 요인으로 자살시도(B=0.135, p<.001), 우울감(B=0.039, p<.001), 음주(B=0.216, p<.001) 등이 확인되었다. 결론적으로 청소년 흡연은 정신건강과 밀접하게 연관되어 있으므로 정신건강 증진을 포함한 통합적 접근이 필요하다.

영문 초록

This study aimed to identify differences in mental health status between adolescents with and without a history of smoking. It also compared these mental health outcomes between middle and high school students. This cross-sectional study was a secondary analysis of data from the 2022 Korea Youth Health Behavior Survey. A total of 48,666 (26,182 middle school and 22,484 high school) were included, and data were analyzed using chi-square tests and multiple regression. Adolescents who had ever smoked exhibited significantly higher rates of depressed mood, loneliness, insufficient sleep, suicidal ideation, and suicide attempts compared to their non-smoking peers (suicide attempts: 6.9% vs. 1.6%, p < .001). Additionally, high school students were more likely than middle school students to report these adverse mental health outcomes. In multiple regression analysis, a history of suicide attempts (B = 0.135, p < .001), depressed mood (B = 0.039, p < .001), and alcohol use (B = 0.216, p < .001) emerged as the strongest factors associated with adolescent smoking behavior. In conclusion, adolescent smoking is closely associated with poor mental health, highlighting the need for integrated approaches that include mental health promotion.
