국문 초록

This study aims to evaluate the public health and economic impacts of the Pfizer-BioNTech XBB.1.5 monovalent vaccine against the Omicron XBB variant for older adults aged 65+ years in South Korea. We adapted a combined Markov-Decision tree model consisting of susceptible, recovered, vaccinated, infected, and death states. The populations that transitioned to the infected state are entered into the decision tree model, which consists of clinical events related to omicron infection. The input data, including infection rate, the probability and mortality of clinical events, vaccine efficacy and coverage, medical costs, and utility were obtained from national statistical database and published literature. In the total population (n=9,818,975), compared to no vaccination, the Pfizer-BioNTech XBB.1.5 monovalent vaccine prevented 249,731 symptomatic infections, 200,898 outpatient cases, and 48,832 hospitalizations, averting 49,000 life-years (LYs) lost and 5,553 quality-adjusted life-years (QALYs) lost. For the medical costs, XBB.1.5 monovalent vaccine saved US$ 168,077,821 on COVID-related clinical events. This study showed that the XBB.1.5 monovalent vaccine can substantially prevent SARS-CoV-2 infections and related clinical events, resulting in improved QALYs and reduced clinical event costs. Our results will provide valuable evidence to support clinical and policy decisions regarding COVID-19 vaccination strategies.
