국문 초록

목적: 본 연구는 외상성 뇌손상이 있는 성인 장애인의일상생활활동 영역 중 삶의 질에 가장 큰 영향을 미치는영역을 알아보고자 하였다. 방법: 본 연구는 2017년 장애인 실태조사의 원시자료를이용하였다. 대상자 선정은 전체장애인 6,549명 중 19세 이상성인이며, 외상성 뇌손상이 있는 사람 46명을 선택하였다. 대상자의 인구사회학적 특성 6개, 기본적 일상생활활동 12 개 영역, 수단적 일상생활활동 8개 영역, 삶의 질(EuroQol-5 Dimension, EQ-5D)이 평가되었다. 종속변수는 EQ-5D 총점이었다. 독립변수는 모형 1이 기본적 일상생활활동 영역이었고, 모형 2가 수단적 일상생활활동 영역이었다. 모형3은 질병 원인을 제외한 모든 변수가 포함되었다. 통계분석은 단계적 다중회귀분석을 사용하였다. 결과: 모형 1에서, 옷 갈아입기(p=.004)와 이동(p=.005)이삶의 질에 영향을 미쳤다(R²=.760). 모형 2에서, 대중교통수단 이용(p<.001)이 삶의 질에 영향을 미쳤다(R²=.695). 모형3에서, 옷 갈아입기(p=.015)와 이동(p=.002), 장애등급(p= .042)과 발병기간(p=.045)이 삶의 질에 영향을 미치는 것으로 나타났다(R²=.809). 결론: 본 연구의 결과는 작업치료사는 외상성 뇌손상이있는 성인 장애인의 일상생활활동 증진을 위해 장애등급과발병기간을 고려해야하며, 일상생활활동 향상을 위한 중재프로그램은 옷 갈아입기와 이동, 대중교통수단 이용 능력향상에 초점을 두어 개발할 필요가 있음을 제안한다.

영문 초록

This study aimed to examine the influence of activities of daily living on quality of life in disabled adults with traumatic brain injury. The study utilized raw data from the 2017 National Survey of Disabled Persons. From 6,549 individuals with disabilities, 46 adults aged ≥19 years with traumatic brain injury were selected as participants. Measurements included six sociodemographic characteristics, 12 domains of basic activities of daily living, eight domains of instrumental activities of daily living, and quality of life assessed using the EuroQol-5 Dimension (EQ-5D). The dependent variable was the total EQ-5D score, while the independent variables comprised sub-domains of basic activities of daily living (Model 1) and instrumental activities of daily living (Model 2). Model 3 included all variables except etiology. Stepwise multiple regression analysis was employed for statistical analysis. In Model 1, dressing (p=.004) and transfer (p=.005) affected EQ-5D (R²=.760). In Model 2, public transportation use (p<.001) affected EQ-5D (R²= .695). In Model 3 (R²=.809), factors affecting EQ-5D were dressing (p=.015), transfer (p=.002), disability grade (p=.042), and onset duration (p=.045). These findings suggest that occupational therapists should consider disability grade and onset duration when designing interventions to enhance activities of daily living in disabled adults with traumatic brain injury. Intervention programs should prioritize improving dressing, transfer, and public transportation use to promote a better quality of life in this population.
