자폐스펙트럼장애 아동의 성별, 출생체중, 재태 연령, 출생 방법에 따른 자폐 증상 심각도 차이

국문 초록

목적 본 연구는 자폐스펙트럼장애로 진단받은 아동을 대상으로 성별, 출생체중, 재태 연령, 출산 방법과 아동의 적응 행동 수준, 자폐 증상 심각도 차이를 알아보고자 실시된 후향적 분석 연구이다. 방법 정신과 전문의로부터 자폐스펙트럼장애로 진단받은 71명의 아동을 대상으로, 대상 아동에게 K-Vineland-II 검사와 K-CARS2 -ST 검사를 시행하여 각 변인의 빈도 및 기술통계분석, 상관 분석, 다중회귀분석을 실시하였다. 결과 성별에 따른 차이에서는 K-CARS2-ST 검사에서만 여아에서 더 높은 점수가 확인되었다(p=0.004). 출생체중, 출생 방법에서는 두 검사에서 유의미한 차이가 발견되지 않았다. 재태 연령에서는 K-CARS2-ST 검사에서 약한 음의 상관관계가 확인되었다(r=-0.282, p=0.022). 결론 본 연구에서 아동이 태어나는 순간 결정되는 요인들과 아동이 이후에 보이는 적응 행동의 수준이나 자폐 행동의 심각도 사이에는 통계적으로 유의미한 상관관계가 없음을 입증하였다. 이로써 양육과 교육, 치료에 더 많은 관심과 노력이 필요하다는 주장의 근거 기반으로 활용될 수 있다.

영문 초록

Objectives This study is diagnosed to find out differences in child's level of adaptive behavior and symptom se-verity according to gender, birth weight, gestational age, and type of birth. Methods This study is a retrospective analysis of 71 children with autism spectrum disorder. K-Vineland-II and K-CARS2-ST tests were performed, and descriptive analysis of each variable, correlation analyses, and multiple regression analysis were performed. Results In terms of gender differences, higher scores were found in girls only in the K-CARS2-ST test (p=0.004). No significant difference was found in the two tests in birth weight and type of birth. A weak negative correlation was found in the K-CARS2-ST test in the gestational age (r=-0.282, p=0.022). Conclusions This study demonstrated that there was no statistically significant correlation between the factors determined at the moment a child is born and the level of adaptive behavior or the autism symptom severity that the child shows afterwards. This can be used as a basis for the argument that more attention and effort are needed for parenting, education, and treatment for the children with autism spectrum disorder.
