국문 초록

목적 본 연구에서는 제한형 신경성 식욕부진증(AN-R)의 발병을 예측하고 치료 개시에 영향을 미치는 증상 변인들을 선정하고, 심리학 연구에 있어서 소셜 미디어 댓글 연구의 유용성을 확인해 보았다. 방법 이를 위하여 섭식장애 관련 한국어 유튜브 동영상 333개에서 AN-R을 진술하는 댓글들을 수집하였고, ChatGPT의 분석을거쳐 1,210개의 증상 증거들을 추출하였으며, 심리적, 신체적, 행동적, 사회적 증상으로 분류한 뒤, α=0.5에서 회귀분석을 실시하였다. 결과 그 결과, 심리적 증상과 신체적 증상 간의 부적 상관을 확인하였다. 심리적 증상의 수준은 AN-R 발병 초기에 가장 높다가신체적 증상들의 발현과 함께 점차 낮아졌다. 증상의 중증도에 관한 자기인식 수준은 신체적 증상과 사회적 증상, 특히 외관의 변화등 외적인 요인에 의해 주된 영향을 받았다. 한편, 치료 개시는 외관의 변화와는 정적 상관을, 심리적 증상과는 부적 상관을 보였다. 결론 이를 통해, 심리적 증상이 고도화된 상태에서 신체적 증상이 발현되는 양상이 나타날 때 AN-R의 발병이 예측된다는 결론과함께, 치료 개시에 있어서 심리적 증상의 지배적 영향력을 보고한 선행연구의 결과를 재확인하였다.

영문 초록

Objectives This study aimed to predict the onset of Restrictive Anorexia Nervosa (AN-R) and identify influential symptom variables for treatment initiation. Methods We collected 333 comments from Korean YouTube videos related to eating disorders, focusing on AN-R. Through ChatGPT assessment, we extracted 1,210 symptomatic pieces of evidence, categorizing them into psychological, physical, behavioral, and social symptoms. Regression analysis at α=0.5 revealed a negative correlation between psychological and physical symptoms. Results Psychological symptoms were highest at the early onset of AN-R, gradually decreasing as physical symptoms emerged. Self-perception of symptom severity was primarily influenced by physical and social symptoms, especially appearance changes. Treatment initiation showed a positive correlation with appearance changes and a negative correlation with psychological symptoms. Conclusions Our findings confirm that AN-R onset can be predicted when physical symptoms appear after progressing psychological symptoms, highlighting the dominant influence of psychological symptoms in treatment initiation.
