[종설] 롱코비드의 기능의학적 접근과 관리

국문 초록

‘코로나 후유증(long COVID, 롱코비드)’은 급성 코로나19 감염증에서 회복된 이후에도 상당기간 지속되는 다양한 증상과 합병증을 뜻한다. 대표적인 증상들로 피로감, 두통, 미각 및 후각 이상, 만성 기침, 호흡곤란, 집중력 및 기억력 저하, 수면 장애, 우울 및 불안, 탈모 등이 있으며, 롱코비드의 발생률은 연구 결과에 따라 차이가 있지만 코로나19 확진자의 20%-60% 정도로 보고되고 있다. 하지만 이러한 높은 발생률과 의학적, 사회적 부담에도 불구하고 아직 롱코비드에 대한 명확한 정의나 객관적인 진단 기준이 없고, 의학적인 검사 및 치료도 연구나 근거가 부족한 상황이다. 하지만 기능의학적으로 부신기능 저하, 장 건강 이상, 염증 회복, 히스타민 불균형, 비타민 D 부족 등의 측면에서 롱코비드를 접근하여 혈액검사, OAP, 음식 불내성 검사 등을 활용하고 적극적인 생활습관 교정 및 경구, 주사 영양치료를 시행한다면 환자에게 도움을 줄 수 있다. 따라서 본 원고에서는 지금까지 밝혀진 롱코비드의 의학적인 자료들을 정리하고, 기능의학적으로 롱코비드를 접근, 관리하는 방법에 대해 제시하고자 한다.

영문 초록

Long COVID, also known as post-acute sequelae of SARS-CoV-2 infection or post-acute COVID-19 syndrome, refers to a range of symptoms and complications that persist more than 4 weeks after diagnosis. Common symptoms include fatigue, headache, anosmia, ageusia, chronic cough, shortness of breath, impaired concentration and memory, sleep disturbances, depression, anxiety, and hair loss. The incidence of long COVID varies according to the data, but it has been reported in 20% to 60% of acute COVID-19 patients. Despite its high incidence and significant medical and societal burden, there is currently no clear definition or objective diagnostic criteria for long COVID. Its diagnosis and treatment options are also limited due to insufficient evidence. Nevertheless, a functional medicine approach, through aspects such as adrenal fatigue, gut health, inflammation resolution, histamine imbalance, and vitamin D deficiency, can help long COVID patients. Blood tests, urinary organic acid profile, and food intolerance tests can be utilized, and lifestyle modification, oral or intravenous nutrition therapies can be implemented. In this article, we review the existing medical literature on long COVID and propose a functional medicine approach and management for long COVID.
