오타모반에서 동반된 시신경유두 색소 침착 1예

국문 초록

목적: 오타모반에서 동반된 시신경유두 색소 침착 1예를 보고하고자 한다. 증례요약: 56세 여자 환자가 좌안 녹내장 의심 소견으로 내원하였다. 안과적 병력은 없었다. 최대교정시력은 양안 0.7이었고, 안압은사용하는 안압하강제 없이 양안 15 mmHg였다. 세극등현미경검사에서 우안 공막에 오타모반이 관찰되었고, 홍채 색소가 증가된 소견이 관찰되었다. 전방각경검사상 우안에서 섬유주 색소 침착, 안저검사상 우안에서 맥락막 및 시신경유두에 색소 침착을 보였고, 빛간섭단층촬영에서 맥락막두께가 반대안에 비해 두꺼워져 있었으나, 녹내장성 시신경 손상 소견은 없었다. 좌안은 특이 소견을 보이지않았다. 결론: 시신경유두 색소 침착이 오타모반 환자에서 관찰될 수 있음을 보여주는 국내 최초 증례로 생각하여 이를 보고하는 바이다.

영문 초록

Purpose: To report a case of optic disc pigmentation in a nevus of Ota patient. Case summary: A 56-year-old female was referred to our clinic with suspected glaucoma in the left eye. She had no previous history of ocular surgery or treatment. The best corrected visual acuity and intraocular pressure were 0.7 and 15 mmHg in both eyes, respectively. Slit lamp microscopy demonstrated nevus of Ota in the right sclera and increased iris pigmentation. Gonioscopy demonstrated increased trabecular meshwork pigmentation in the right eye. Fundus examination showed pigmentation of the choroid and optic disc in the right eye; choroidal thickness of the right eye was greater than that of the left eye on optical coherence tomography. However, there were no glaucomatous changes in the right eye. There were no specific findings in the left eye. Conclusions: To the best of our knowledge, this was the first reported case of optic disc pigmentation in a Korean nevus of Ota patient.
