각막굴절교정렌즈 처방의 원리와 치료 효과

국문 초록

각막굴절교정학(orthokeratology)이란 특수하게 디자인된 산소투과경성(rigid gas permeable [RGP]) 콘택트렌즈를 이용하여 중심각막을 편평하게 하여 근시와 난시를 일시적으로 교정하는 방법이다. 그 교정 범위는 약 -2.50 diopters (D)에서 -4.00 D의 교정 효과를 치료 시작 10일 정도 후에 나타낸다. 디자인은 편평한 base curve radius (BCR)과 아울러서 가파른 secondary curve radius (reverse curve)이 편평한 주변부 커브(peripheral curve radius)를 담당하는 얼라인먼트 커브(alignment curve)와 연결시켰는데, 이러한 디자인은 렌즈가 주변부 각막에 정렬할 수 있게 도와주어 중심안정을 향상시키며 렌즈의 안정성을 높이고, 심지어 매우 편평한 중심 BCR에서도 가능하게 하였다. 원시나 노안에 대한 각막굴절교정렌즈도 개발되어 있고, 또한 치료 영역의 넓이를 줄이고 기본만곡에 비구면도를 가입하며 역기하커브와 정렬커브 디자인을 조정하여 고도근시에도 영역이 확대되고 있다. 이렇듯 새로운 영역의 각막굴절교정렌즈의 개발에 따라 앞으로도 점차 활용도와 피팅 영역이 확대될 것으로 기대된다.

영문 초록

Orthokeratology is the way to correct the myopia or astigmatism by flattening or the central cornea with specialty lenses. The range of correction is from -2.50 to -4.00 diopters after 10 days of fitting. The designs is constructed with flat base curve radius, steep reverse curve, flat alignment curve, and peripheral curve. This multi-cuve design enabled the orthokeratology lenses to stay on the cornea stably and effectively. Recently, the application of orthokeratology is extending to hyperopia and presbyopia. In the future, the amount of correction and the application of orthokeratology will increase more and more.
