백내장 수술 시 CORNEALⓇ Lens를 삽입한 고도근시안에서의 술 후 굴절력 예측

국문 초록

목적: 고도근시를 가진 백내장 환자에서 CORNEALⓇ lens를 사용하여 후방 인공수정체 삽입술을 시행한 후 수술 전 예상굴절력과 수술 후 실제굴절력을 비교하고자 하였다. 대상과 방법: 고도근시 백내장 환자 26명 41안을 대상으로 수술 후 1주, 1개월, 3개월, 6개월째 자동굴절검사계와 현성굴절력을 병행하여 굴절상태를 측정하였으며, 수술 전 인공수정체 도수 산정은 SRK/T공식을 사용하였다. 수술 전 예상굴절력과 수술 후 실제굴절력의 차이는 일원변량분석을 실시하였으며, 사후검증으로 Duncan을 사용하여 유의성 여부를 판단하였다. 결과: 수술 3개월 후 측정된 spherical equivalent (SE) 값은 예측치에 비해 원시변화를 보였다. 술 후 1주, 1개월, 3개월, 6개월째 SE 값은 각각 －2.76±0.26 D, －2.24±0.21 D, －1.89±0.20 D, －1.73± 0.23 D로 시간이 지남에 따라 유의하게 원시쪽으로 치우치는 양상을 보였다(p＜0.05). 결론: 고도근시를 가진 백내장 환자에서 CORNEALⓇ lens를 사용하여 후방 인공수정체 삽입술 시 예상굴절력보다 다소의 원시변화가 있는 것으로 생각한다.

영문 초록

Purpose: To compare preoperative estimated refraction and postoperative actual refraction after posterior chamber lens implantation with CORNEALⓇ (Pringy, France) lens in high myopia during cataract surgery. Methods: The 41 eyes (26 cataractous patients in high myopia) were evaluated from April 2000 to December 2004. Postoperative 1 week, 1 month, 3 months, 6 months, actual refraction was measured objectively by autokerato-refractometer and manifest refraction. And preoperative predicted refraction was estimated using SRK/T formula. The difference between preoperative predicted refraction and postoperative actual refraction was verified using one way ANOVA, the average difference verification. And the significancy was estimated by Duncan. Results: Postoperative 3 months, the spherical equivalent seemed to be hyperopic. 1 week after surgery, the spherical equivalent was －2.76±0.26 D, 1 month, 3 month, and 6 month after surgery, the spherical equivalent was 2.24±0.21 D, －1.89±0.20 D, and －1.73±0.23 D. As time goes by, it tended to be moved hyperopia significantly (p＜0.05). Conclusions: Postoperative predicted refraction in high myopia implantated with CORNEALⓇ lens during cataract surgery seems to migrate hyperopia to some degree compared to the preoperative estimated refraction.
