눈물샘에 발생한 아포크린땀샘낭종

국문 초록

목적: 눈물샘에 발생한 아포크린땀샘낭종 1예를 경험하였기에 이를 보고하고자 한다. 증례요약: 51세 여자 환자가 내원 2년 전에 처음 발생된 좌측 위눈꺼풀 종괴를 주소로 내원하였다. 초진 시 좌측 위눈꺼풀 외측의 눈물샘 부위에 만져지는 무통성 종괴가 관찰되었으며, 세극등현미경검사에서 좌측 위눈꺼풀 결막에 경계가 잘 지어진 분홍색 종괴가 관찰되었다. 안와 전산화단층촬영에서 좌안의 상이측 결막에 8 x 9 x 9 mm 크기의 조영증강이 된 비교적 경계가 분명한 종괴가 관찰되었다. 국소 마취하에 좌측 위눈꺼풀의 눈물샘 부위에 발생한 낭성 종괴에 대해 절제 생검술을 시행하였고, 병리조직학검사에서 아포크린땀샘낭종으로 확인되었다. 술 후 1년까지 재발 소견은 관찰되지 않았다. 결론: 아포크린땀샘낭종은 눈물샘 부위에 발생하는 경우가 극히 드물어 국내에서는 첫 보고이며, 눈물샘 부위 종괴의 감별진단에서 고려되어야 하겠다.

영문 초록

Purpose: To report a case of apocrine hidrocystoma in the lacrimal gland. Case summary: A 51-year-old woman presented with a left upper eyelid mass that occurred two years before presenting to hospital. Upon physical examination, a painless palpable mass on the left upper eyelid was observed. Based on slit-lamp examination, a pinkish mass with a well-circumscribed border on the left upper temporal conjunctiva was observed. Orbital computed tomography showed a well-defined contrast enhanced mass of 8 × 9 × 9 mm in the lacrimal lesion. The mass was removed completely under local anesthesia. Biopsy confirmed that the mass was an apocrine hidrocystoma. There was no recurrence at one year postoperatively. Conclusions: This is the first report of an apocrine hidrocystoma in a lacrimal lesion in Korea. Apocrine hidrocystoma is rarely found on lacrimal gland lesions, but it should be considered in the differential diagnosis of a lacrimal gland mass.
