의안 장기착용자에서의 의안 관리

국문 초록

목적: 1년 이상 의안을 착용하고 있는 무안구증환자에서 무안구증 눈과 정상 눈을 비교하여 의안 관리에 도움을 주고자 하였다. 대상과 방법: 한눈 안구제거술 및 안와충전물삽입술 후 1년 이상 의안을 착용한 총 40명을 대상으로 무안구증 40안과 정상 40안에 대해 의무기록을 후향적으로 분석하였다. 성별, 나이, 눈물점 이상 여부, 토끼눈, 눈 분비물의 상태, 눈물띠 높이, 위눈꺼풀결막의 상태, 마이봄샘 기능부전 동반 여부, 쉬르머검사, 눈물소관관류술, 진단적 더듬자검사, 의안착용기간, 의안 제거 빈도 결과에 대해 조사하고 의안을 착용하는 무안구증 눈을 정상 눈과 비교하였다. 결과: 의안을 착용하는 무안구증 눈은 토끼눈이 있는 군이 그렇지 않은 군에 비하여 눈의 분비물이 생기는 빈도가 더 많았으며 (p=0.04), 의안을 자주 제거하는 군이 그렇지 않은 군에 비하여 눈의 분비물이 생기는 빈도가 더 많았다(p=0.04). 토끼눈의 동반 여부, 의안 제거 빈도는 눈물띠 높이, 쉬르머검사, 눈물소관관류술 결과와는 큰 연관성이 없었다(0.174

영문 초록

Purpose: To aid in the prosthesis care by comparing anophthalmic eyes with normal eyes in anophthalmic patients wearing prosthetic eyes for more than 1 year. Methods: We retrospectively reviewed the medical records of 40 anophthalmic eyes and 40 normal fellow eyes of 40 patients who wore prosthetic eyes for more than 1 year after unilateral eye removal and orbital implant insertion. We compared data obtained from medical records on sex, age, punctal abnormality, lagophthalmos, eye discharge characteristics, tear meniscus height, upper eyelid palpebral conjunctiva, Meibomian gland dysfunction, Schirmer’s test, lacrimal irrigation test, diagnostic probing, period of wearing the prosthetic eye, and frequency of prosthesis removal between anophthalmic eyes wearing a prosthesis and normal eyes. Results: Anophthalmic eyes with lagophthalmos wearing a prosthesis had a higher frequency of eye discharge than eyes without lagophthalmos (p = 0.04). Eyes with frequent prosthesis removal had a higher frequency of eye discharge than eyes with less frequent prosthesis removal (p = 0.04). However, lagophthalmos and prosthesis removal frequency were not associated with tear meniscus height, or the results of Schirmer’s test, or the lacrimal irrigation test (0.174 < p < 0.971). Conclusions: The frequency of prosthesis removal and presence of lagophthalmos can affect the discharge of anophthalmic eyes. It is advisable not to remove the prosthesis more than once a month to minimize eye discharge. J Korean Ophthalmol Soc 2019;60(3):217-222
