위암 진단 목적으로 복부CT검사 시 우 측와위 자세에서의 병변 검출 유용성에 관한 연구

국문 초록

목적 본 논문에서는 경구 조영체인 물을 마시고 복부CT를 촬영하는 환자 중 우 측와위 자세에서의 위암 발견 유용성을 알아보고자 시행하였다. 대상 및 방법 2010년 7월부터 2011년 12월까지 내시경검사에서 위암의 소견을 보였고, 수술 전 위암 진단 목적으로 물을 먹고 복부CT를 시행한 환자 중 병리조직 소견상 위암으로 확진된 33명의 환자를 대상으로 후향적 분석을 하였다. 위암의 발생빈도가 높은 위 체부, 전정부 부분을 CT검사 시 우 측와위 자세에서 병변 발견 여부의 적합성을 3단계 평가구조를 이용하여 앙와위 자세와 비교 평가하였다. 본 연구는 내시경 및 수술 후 병리검사 결과와 비교하였다. 결과 우 측와위 자세에서 위암 발견 능력의 평균값은(1.97±0.847)로 앙와위 평균값인(1.63±0.652)보다 높게 나왔다. 그리고 우 측와위 자세에서는 전정부 내강의 크기(3.55±1.281)으로 앙와위 자세(3.05±1.138)보다 더 크게 관찰되었다 또한 우 측와위 자세에서는 위암의 크기가(2.83±1.895)으로 앙와위 자세(2.19±1.420)보다 더 자세히 관찰되어 병리결과에 가까웠다. 결론 위암 수술을 하지 않은 환자의 복부CT검사에서의 우 측와위 자세는 환자의 위 체부와 전정부 부위의 병변 진단에 있어서 효율성을 높일 수 있는 유용한 검사라고 사료된다.

영문 초록

I. Purpose The purpose of this study was carried out to examine the usefulness of the detection of gastric cancer at the Rt Lateral Decubitus position of a patient who has an abdominal CT scans after drinking water, an oral contrast agent. II. Meterial and Methods A retrospective analysis was carried out for 33 patients who showed a symptom of gastric cancer in endoscopy from July 2010 to December 2011 and got confirmed the diagnosis of gastric cancer on pathology organization opinion among those who got abdominal CT scans after drinking water for the purpose of diagnosing gastric cancer before operation. The fitness of the detection of lesions at the Rt Lateral Decubitus position in CT scans on the body of stomach and the antrum region with high occurrence of cancer of the stomach was compared and evaluated to that of the supine position using three-stage evaluation structure. This study made a comparison with the result of pathological examination after endoscopy and surgery. Ⅲ. Result The average value of the gastric cancer detectability at the Rt Lateral Decubitus position was 1.97±0.847, which was higher than the average value of that at the supine position (1.66±0.692). And at the Rt Lateral Decubitus position, the size of the antrum wall was 3.55±1.281, which was bigger than that of the supine position(3.05±1.138). Also, at the Rt Lateral Decubitus position, the size of gastric cancer was 2.83±1.895, which was bigger than that of the supine position (2.19±1.42), so it was closer to the pathological results. IV. Conclusions Rt Lateral Decubitus position at the abdominal CT scans of a patient who has not gone through a gastric cancer surgery is thought to be a useful inspection that can increase the efficiency of the diagnosis of the patient’ s body of stomach and antrum region.
