신규간호사를 위한 시뮬레이션 기반 인수인계 교육요구 분석 및 시나리오 개발

국문 초록

본 연구의 목적은 신규간호사가 가장 어려워하는 업무 중 하나인 인수인계 능력 향상을 위하여 시뮬레이션 기반 인수인계 교육요구를 분석하고 시나리오를 개발하는 것이다. 연구기간은 2015년 2월부터 5월까지이며, 첫 번째 과정은 포커스그룹 인터뷰를 통해 인수인계 교육요구의 내용을 분석하는 것으로 신규간호사 6명, 프리셉터 간호사 4명과 간호관리자 4명이 참여하였다. 내용분석 방법으로 3개 주제와 10개 소주제를 파악하였고, 3개 주제는 환자상태 파악, 인수인계 기본 지식과 임상판단력이었다. 두 번째 과정은 22개 문항의 인수인계 교육요구도 설문조사를 시행하였으며 신규간호사, 프리셉터간호사와 간호관리자가 각각 10명이 참여하였다. 교육요구가 높게 나타난 항목은 환자의 주호소나 상태의 중대한 변화 인계, 새로 입원한 환자 인계, 검사나 수술 전후 준비 인계, 주요 비정상적인 검사결과에 대한 인계의 순서로 나타났다. 세 번째 과정은 교육요구도가 높게 나타난 내용을 반영하여 간호학 교수 2명과 간호관리자 2명이 참여하여 시뮬레이션 기반 인수인계 교육의 시나리오로서 호흡곤란 대상자의 간호 및 인계 사례를 개발하였고, 교육 목적에 따라 적절하게 전개되는 것을 확인하였다. 인수인계에 직접 참여하는 학습자 중심의 교육요구도를 바탕으로 인수인계 교육 방법을 수립할 때에 본 연구의 결과를 활용할 수 있을 것으로 기대한다.

영문 초록

The aim of this study was to assess educational needs on nursing handoffs for new nurses and de v elop scenarios for the simulation-based education on nursing handoffs that new nurses consider as one of the most difficult tasks. Research period is from August 1 to November 30, 2015. The participants were 6 new nurses, 4 preceptor nurses and 6 nurse managers for the focus group inter view in the first research process, three topics were identified as the handoffs education contents: assessment of a patient ’ s condition, basic knowledge of nursing handoffs, and clinical judgment. Another 10 new nurses, 10 preceptor nurses, and 10 nurse managers for the evaluation of the importance of the education contents were participated in the second research process. The contents for the simulation-based nursing handoffs education were evaluated with more than .80 of content validity index, they were the handoff of a patient ’ s chief complaints or significant changes in a patient ’ s condition; the handoff of a new patient; the hand off of testing and perioperati v e preparation; and the handoff of the major abnormal test results of a patient (in order of highest needs to lowest). Based on such needs, two nursing professors and two nurse managers analyzed clinical cases and de v eloped the scenarios on nursing care and handoffs for patients with dyspnea. In addition, we confirmed that the scenarios was conducted according to learning objectives for nursing handoffs. We hope that the results in the present study will be useful for the development of learner-centered education programs on nursing handoffs.
