국문 초록

목적: 유리체내 아플리버셉트(aflibercept) 주사 후 발생한 비감염성 안내염을 보고하고자 한다. 증례요약: 67세 남자 환자가 우안의 습성 나이관련 황반변성으로 유리체내 aflibercept 주사 2일 후 시작된 경미한 통증이 동반된 시력저하를 주소로 내원하였다. 세극등 현미경 검사에서 결막충혈은 경미하였고 전방 및 유리체내 중등도의 염증세포소견이 관찰되었으나 통증 및 전방축농은 없었다. 비감염성 안내염으로 진단하였고 점안 항생제 및 점안스테로이드를 사용하였다. 치료 3일 후 전방 내 염증세포는 빠르게 감소하는 양상이었고, 치료 10일 후 전방 및 유리체내 염증 소견은 관찰되지 않았으며, 교정시력이 0.1로 이전의 시력만큼 회복되었다. 결론: 유리체내 aflibercept 주사 후 발생한 비감염성 안내염은 점안 스테로이드 치료를 통해 비교적 빠른 호전을 기대할 수 있다. <대한안과학회지 2016;57(6):1018-1021>

영문 초록

Purpose: To report a case of noninfectious endophthalmitis after intravitreal injection of aflibercept. Case summary: A 67-year-old male patient presented with sudden vision loss 2 days after intravitreal aflibercept injection for neovascular age-related macular degeneration. Mild conjunctival injection and moderate inflammation in the anterior chamber were noted, but he had no ocular pain or hypopyon. The patient was diagnosed with noninfectious endophthalmitis and successfully treated with topical antibiotics and topical steroid eye drops. The infection was apparently cleared at 3 days. At 10 days after the initial presentation, his vision loss resolved completely. Conclusions: Noninfectious endophthalmitis after intravitreal injection of aflibercept can be treated with topical steroids. J Korean Ophthalmol Soc 2016;57(6):1018-1021
