비에띠결정색소망막이영양증에서 나타난 바깥 망막 관상화 3예

국문 초록

목적: 비에띠결정색소망막이영양증 3예, 6안의 Outer Retinal Tubulation (ORT) 유병률과 그 특징에 대하여 보고하고자 한다. 증례요약: 비에띠결정색소망막이영양증으로 진단된 환자 3명(6안)에서 천연색안저사진, 안저자가형광촬영, 빛간섭단층촬영, 형광안저혈관조영, 망막전위도, 눈전위도를 실시하였으며 모든 환자에서 ORT가 관찰되었다. 6안에서 관찰된 ORT는 스펙트럼영역 빛간섭단층촬영의 B-scan 상에 바깥의 고리는 과반사를 나타내었고 내부는 저반사를 나타내었으며 내부 공간에 과반사의 반점들이 관찰되었다.ORT는 중심와를 포함하여 중심와 바깥에서 관찰되었고, 바깥핵층에 위치하였다. ORT의 크기는 70×140 μm 이상을 벗어나지 않았다. 결론: ORT는 비에띠결정색소망막이영양증에서 모두 관찰되었으며, 이러한 소견은 비에띠결정색소망막이영양증에서 망막색소상피의 위축과 함께 광수용체에도 광범위한 이상이 있음을 말해 준다.

영문 초록

Purpose: To report the prevalence and the characteristics of outer retinal tubulation (ORT) in Bietti’s crystalline dystrophy in 6eyes of 3 patients. Case summary: Three patients with Bietti’s crystalline dystrophy were examined using color fundus photography, fundus autofluorescence picture, spectral domain optical coherence tomography (SD-OCT), fluorescein angiography, electroretinogram and electrooculogram. ORT was detected in 3 of 3 (100%) Bietti’s crystalline dystrophy patients. SD-OCT B-scan revealed hyperreflective material inside the hyporeflective internal space with hyperreflective border. ORT was observed under the fovea or outside the fovea and on the outer nuclear layer in the retina. ORTs were no greater than 70 x 140 μm in size when measured using SD-OCT B-scan image. Conclusions: ORT was observed frequently in patients with Bietti’s crystalline dystrophy. These findings can potentially predict the extensive photoreceptor abnormalities in retinal pigment epithelial atrophy.
