A Case of Progressive Multifocal Leukoencephalopathy in a 10-year-old Girl

국문 초록

진행성 다발성 백질뇌병증은 JC 바이러스의 감염에 의해 발생하는 중추신경계의 염증성 질환으로, 뇌백질의 다병소에 진행성 염증을 일으키며, 예후가 나쁜 것으로 알려져 있다. 주로 AIDS 환자와 같이 면역 기능이 저하된 환자에 호발하지만, 혈액 종양 환자, 장기 이식 환자 등에서도 발견된다. 본 증례는 10세 여아로 발열과 호흡곤란을 주소로 본원에 내원하였다. 환아는 반복적인 호흡기 감염의 병력이 있었으며 만 6세에 원발성 섬모운동 이상증을 진단받았고, 당시 시행한 림프구 분석 검사에서 CD19+ 세포의 감소 소견이 확인되었다. 입원 이후 환아는 우측 상하지의 근력 저하가 점차 진행되었다. 뇌 자기공명영상을 시행하였으며, T2 강조영상에서 중간뇌, 뇌교, 숨뇌 및 양측 대뇌각 등 다양한 병변에서 고신호강도를 보이고, T1 강조영상에서 저신호강도를 보였다. 또한 우측 소뇌가 좌측 소뇌 반구 쪽으로 부분적인 돌출을 보였다. 감염 및 종양, 자가면역 질환을 감별하기 위해 시행한 혈액 검사에서는 특이 소견을 보이지 않았다. 소뇌 병변에서 시행한 조직검사에서 JC 바이러스에 의한 진행성 다발성 백질뇌병증이 확인되었다. 환아는 입원 59병일째 패혈증으로 인한 급성 호흡 곤란 증후군으로 사망하였다.

영문 초록

Progressive multifocal leukoencephalopathy (PML) is a fatal viral disease caused by the John Cunningham virus (JCV) and is characterized by progressive inflammation of the white matter at multiple locations in the brain. A 10-year-old girl with repeti-tive histories of frequent respiratory tract infections was admitted to our hospital due to dyspnea and fever. She had been diagnosed with primary ciliary dyskinesia at 6 years of age, and a lymphocyte analysis at that period revealed a decreased CD19+ cell count. During admission, she developed a motor deficit of the right leg and arm with gradual aggravation. Magnetic resonance imaging showed high signal intensity on the T2-weighted images and low signal intensity on the T1-weighted images in the lower mid-brain, pons, medulla, both cerebral peduncles and right cerebellum with partial extension into the left cerebellar hemisphere. Laboratory tests to identify the diseases of infection, malignancy, and autoimmune origin returned negatives. Biopsy results of the lesion in the cerebellum revealed PML due to JCV. On the 59th day of hospitalization, she expired of acute respiratory distress syndrome due to sepsis.
