국문 초록

본 연구는 건강의 가치가 경제에 미치는 영향을 분석하기 위하여 2001년 5대 사망원인 질병인 암(악성신생물), 뇌혈관질환, 심장질환, 당뇨병, 간질환의 사회경제적 비용추계를 목적으로 한다. 질병부담을 측정하기 위해 비용을 크게 직접비용 및 간접비용으로 구분하였다. 직접비용에는 진료비, 교통비, 간병비 등을 고려하였으며, 간접비용은 인적 자본 접근 방식인 총생산손실계산법을 활용하여 조기사망에 따른 소득 손실액과 입원 및 외래진료로 인한 작업손실비용을 추계하였다. 추계결과, 암이 약 5조 5,954억 원으로 가장 높은 사회경제적 비용이 발생하였으며, 이어 간질환이 1조 9,407억 원, 뇌혈관질환이 1조 7,750억 원, 심장질환이 1조 5,100억 원, 그리고 당뇨병이 9,705억 원으로 분석되었다. 급속한 고령화의 진전과 유병률의 증가에 따라 질병으로 인한 사회경제적 비용은 증대될 것으로 예상할 수 있는바, 본 연구에서는 개인과 가정의 안녕뿐 아니라 지속적인 성장을 위한 국가 전체적인 관점에서 보건수준의 향상을 위한 효율적인 정책수립의 필요성을 강조하였다.

영문 초록

This article estimate socioeconomic costs of five major diseases by causes of death to be consistent with economic theory in studies from a societal perspective. The socioeconomic cost of illness approach focuses on the value of health that may be fairly directly measured in terms of money: medical expenditures and forgone earnings due to illness. We estimates both direct and indirect cost of diseases in Korea in 2001 using human capital approach. Direct costs estimates include medical expenditures, traffic costs, and caregiver's costs. And indirect costs representing the loss of production include lost workdays due to illness and lost earning due to premature death. Consequently, the socioeconomic costs of five major diseases by causes of death in Korea were 11,791 billion Won, which amount to 2% of National Disposal Income (NDI). Specifically, malignant neoplasm was about 5,595 billion Won; hepatism 1,941 billion Won; cerebrovascular disease 1,775 billon Won; heart disease 1,510 billion Won; and diabetes 971 billion Won. This study provides good information for policy maker that government expenditure is allocated to promote population health.
