국문 초록

Objective: To compare the immediate hemodynamic effects of TECAR (thermal energy capacitive and resistive) therapy versus myofascial release (MFR) in adults with lower-extremity edema using Doppler ultrasound indicesᝍpeak systolic velocity (TA max), mean velocity (TA mean), pulsatility index (PI), and resistive index (RI). Methods: In this randomized controlled trial, 26 adults with lower extremity edema were randomly assigned to TECAR or MFR groups. Each intervention was delivered in a single 20-minute session. The TECAR group received sequential CET and RET modes, while the MFR group received manual fascial release techniques applied along the myofascial lines of the affected calf. Peripheral blood flow parameters (TA max, TA mean, PI, RI) were measured using Doppler ultrasound pre- and post-intervention. All measurements were conducted under standardized conditions by a blinded examiner. Data were analyzed using two-way repeated ANOVA (p<0.05). Results: TA max and TA mean increased significantly in both groups (p<0.001). PI decreased in both groups (TECAR p=0.014; MFR p=0.010). RI decreased after TECAR (p=0.011) but not after MFR (p=0.082). Time × group interactions were significant across all indices, and the pre-post change patterns differed between the two groups (all p<0.05). Conclusions: A single session of TECAR therapy or MFR produced immediate improvements in peripheral blood-flow metrics in individuals with lower-extremity edema. TECAR therapy may enhance circulation via deep thermal effects, whereas MFR likely acts through fascial and lymphatic mechanisms. These modalities may be used as complementary options for managing peripheral circulatory impairment.
