매복된 상악 좌측 견치와 전치부 과개교합을 동반한 골격성 II급 부정교합의 발치 치험례

국문 초록

견치의 매복은 상하악 제3대구치 다음으로 흔하게 일어나며 특히 하악보다는 상악에서, 순측보다는 구개측에서 호발한다. 견치 매복의 원인은 여러 가지가 있으며, 영구 견치의 치배 위치 이상에 의해 적절한 맹출 경로를 확보하지 못해 유견치가 만기 잔존되어 있는 경우 유견치의 조기 발거 후 영구 견치의 교정적 견인을 시도할 수 있다. 그러나 교정적, 치주적, 그리고 사회경제적인 요인 등에 의해 부득이하게 매복된 영구 견치의 발거가 필요한 경우가 있으며, 이 경우 제1소구치를 이용하여 임상적, 심미적 그리고 기능적으로 적절하게 견치를 대체해야 한다. 전치부 과개교합의 유병률은 연구에 따라 11.8~36.7%로 보고되며 치료는 과개교합의 발생 원인에 따라 다양한 치료 옵션이 있다. 기준평면에 대한 상악 전치의 수직적 위치와 하악의 curve of Spee, 환자의 안모 형태, 상악 전치 노출도 및 gingival display 등을 고려하여 치성적인 원인에 의한 과개교합으로 판단된다면 임상가는 치료 전 상하악 전치의 압하량과 구치의 정출량을 적절히 결정해야 한다. 본 증례에서는 상악 좌측 견치의 매복과 전치부 과개교합을 동반한 단안모 경향의 II급 부정교합 환자에서 상악 전치의 압하와 함께 상악 우측 제1소구치와 상악 좌측 견치를 발치하였음에도 성공적인 과개교합 개선을 함께 달성하였기에 이를 소개하고자 한다.

영문 초록

Impaction of a permanent canine is the most common form of tooth impaction excluding the maxillary and mandibular third molars. It occurs more in the maxilla than in the mandible and is frequently observed in the palatal side than the labial side.1 If the deciduous canine is over-retained, often with a long and unresorbed root due to a malposition of tooth germ, clinicians could attempt orthodontic traction of the impacted maxillary canine after early extraction of the deciduous canine. However, there are cases in which permanent canines are inevitably removed due to various orthodontic, periodontal, and socioeconomic factors. In these cases, the maxillary first premolar should be used to replace the maxillary canine in a esthetic and functional manner. The prevalence of anterior deep bites is reported to be 11.8% to 36.7%, and there are several treatment options depending on the etiology. If the anterior deep bite was caused by dental issues such as vertical position of upper incisors depending on the reference plane (Horizontal plane; Sella-Nasion plane to 7°), a large curve of Spee of the mandible, maxillary incisor exposure and the gingival display, clinicians should determine the appropriate amount of intrusion needed for the incisors and extrusion of molars before treatment.
