안부 대상포진 후 뇌자기공명영상에서 동안신경 조영증강 소견을 보인 동안신경마비 1예

국문 초록

목적: 안부 대상포진 후 동안신경마비가 속발된 환자의 자기공명영상에서 동측 동안신경의 조영증강이 확인된 증례를 보고하고자한다. 증례요약: 70세 여자 환자가 3일전 발생한 좌안 통증, 어지럼증을 주소로 내원하였다. 좌측 이마와 내안각 주위에 부종, 발적을 보였고동공반사와 안구 움직임은 정상이었다. 안부 대상포진 진단 하에 아시클로버 정주요법을 시작하였다. 일주일 뒤 환자는 갑작스러운복시와 좌안 눈꺼풀 처짐을 호소하였다. 외사시가 관찰되었고 좌안 내전 및 상하전장애와 동공 확장 소견을 보였다. Magnetic resonance imaging (MRI)에서 좌측 동안신경 기시부에 조영증강 소견이 관찰되었다. 내원 2달째, 안구운동장애와 안검하수, 동공산대가 호전되는 양상을 보였다. 내원 4달째, 안구운동장애는 관찰되지 않았고, 정상 눈 위치 소견을 보였다. 안검하수는 완전히 회복되었으며, 동공 크기도 정상화되었다. 결론: 동안신경 마비가 동반된 안부 대상포진 환자에서 MRI상 동측 동안신경의 조영증강이 확인되었다. 이를 통해, 안부대상포진에서속발된 동안신경마비의 병인을 이해하는 데 도움이 될 수 있다. 추후 동안신경마비의 임상 경과에 따른 MRI 소견 변화에 대한 추가연구가 필요하겠다.

영문 초록

Purpose: This report presents a case of oculomotor nerve palsy associated with ipsilateral contrast enhancement of the oculomotor nerve as determined via magnetic resonance imaging (MRI) of a patient with zoster ophthalmicus. Case Summary: A 70-year-old female presented to the ophthalmology clinic with complaints of left eye pain and dizziness that had commenced 3 days prior. Swelling and erythema were observed in the left periorbital area. Both the pupillary light reflex and the eye movements were initially normal. Herpes zoster ophthalmicus was suspected, and intravenous acyclovir was commenced. One week later, the patient developed diplopia and ptosis of the left eyelid. An extraocular motility examination revealed exotropia, impaired medial and vertical gazes, and pupil dilation of the left eye. Contrast-enhanced MRI revealed enhancement of the cisternal portion of the left oculomotor nerve. Ophthalmoplegia, ptosis, and pupil dilation improved after 2 months. By month 4, the ophthalmoplegia had disappeared and the eye position was normal. The ptosis had completely resolved and the pupil had returned to normal size. Conclusions: Contrast enhancement of the oculomotor nerve was apparent in a patient with ophthalmic herpes zoster accompanied by oculomotor nerve palsy. This may enhance our understanding of the etiology of ophthalmic herpes zoster-associated oculomotor nerve palsy. Further research is required to explore the correlations between clinical progression and the MRI findings of such cases.
