Purpose - The purpose of this paper is to evaluate the clinical efficacy of utilizing a free anterolateral thigh (ALT) flap for single-stage reconstruction of extensive postoperative soft tissue defects in patients with scalp scar carcinoma. Design/Methodology/Approach - This study retrospectively analyzes the treatment of scalp scarring cancer patients in our department from September 2021 to December 2023. Focusing on nine cases (4 males, 5 females) aged 37- 59 years, the research examines the outcomes of wide local excision followed by immediate reconstruction with free ALT flaps. Postoperative scalp defects ranged from 11.0×8.0 cm to 21.0×18.0 cm, with donor sites closed directly. Key aspects evaluated during follow-up (1 year) include flap survival, scar formation, functional recovery, and patient satisfaction, providing insights into the effectiveness of this reconstructive approach. Findings - This paper studies the effectiveness of one-stage repair using a free anterolateral thigh flap following the resection of scalp scar carcinoma with large soft tissue defects. By achieving defect coverage while minimizing donor site damage through direct closure, this technique ensures high patient satisfaction and yields favorable functional and aesthetic outcomes. Research Implications - In the management of this study, the findings highlight the significance of one-stage repair using a free anterolateral thigh flap in improving scalp defect reconstruction, enhancing functional and aesthetic outcomes, and minimizing donor site damage. This approach not only benefits patient recovery and satisfaction but also provides a valuable reference for future research in reconstructive surgery.
