Objectives: This study examined the association between weight suppression percentage (WS%) and weight loss rate in overweight and obese adults undergoing a Korean medicine weight management program with Gamitaeeumjowee-tang, and explored preliminary clinical cutpoints. Methods: A retrospective chart review of 824 adults (body mass index [BMI] ≥23 kg/m²) enrolled in a 75-day program at six clinics (November 2022-February 2025) was conducted. WS% and weight loss rate were analyzed using Spearman correlation and restricted cubic spline (RCS) regression. Cutpoints were identified from the RCS curve and validated using receiver operating characteristic (ROC) analysis. Group comparisons used Mann-Whitney U test and analysis of covariance adjusted for age, initial BMI, and menopausal/hysterectomy status. Results: Mean weight loss rate was 7.96±3.65%; 659 participants (80.0%) achieved ≥5% weight loss. WS% showed a significant nonlinear inverse association with weight loss rate (Spearman ρ=-0.147, P<0.001). WS%=0% showed higher weight loss rates than WS% >0% (8.61% vs. 7.57%; Cohen's d=0.3, P<0.001), and WS% ≤4% showed higher rates than WS% >4% (8.18% vs. 7.32%; Cohen's d=0.2, P=0.001), both remaining significant after covariate adjustment. Conclusions: WS% was significantly associated with weight loss outcomes in Gamitaeeumjowee-tang based Korean medicine weight management. The exploratorily derived cutpoints (WS%=0% and 4%) may offer preliminary reference points for individualized clinical approaches in obesity management, though prospective validation is warranted.
