국문 초록

Non-Hodgkin lymphoma (NHL) includes diffuse large B-cell lymphoma (DLBCL) and follicular lymphoma (FL), which differ in clinical characteristics and disease course. This study aimed to compare healthcare utilization and healthcare costs between patients with DLBCL and FL in Korea. A retrospective cohort study was conducted using customized claims data from the Health Insurance Review and Assessment Service (HIRA). Patients were identified based on KCD diagnostic codes and rituximab prescriptions, and were followed for up to 5 years. A total of 3,837 patients with DLBCL and 559 patients with FL were included. Over the 5-year follow-up period, cumulative hospitalization days were longer in DLBCL (87.98 vs. 65.04 days), whereas outpatient visits were more frequent in FL (55.70 vs. 41.85 visits). The 5-year total medical cost was higher in DLBCL (₩60.9 million) than in FL (₩57.5 million). Cost concentration in the first year was greater in DLBCL (71%) than in FL (52%). Medication costs accounted for the largest share of total expenditures in both groups. In generalized linear model analysis, CAR-T therapy, hematopoietic stem cell transplantation, and death were significant cost-increasing factors. These findings suggest distinct patterns of healthcare utilization and economic burden between DLBCL and FL.
